INDICATION

Bofalgan 300mg/2ml Inj is an injectable paracetamol preparation indicated for the short-term treatment of moderate pain and fever, particularly when the oral route is not available or appropriate. It is used for post-operative pain management, fever control in hospitalized patients unable to take oral medications, management of acute pain in the emergency department, and as part of multimodal analgesia in surgical patients to reduce opioid requirements.

MECHANISM OF ACTION

Paracetamol exerts its analgesic and antipyretic effects primarily through central mechanisms. It inhibits cyclooxygenase enzymes (COX-1 and COX-2) in the CNS, reducing prostaglandin synthesis in the hypothalamus and pain-modulating pathways in the brain and spinal cord. It may also activate the endocannabinoid system and descending serotonergic inhibitory pathways. Unlike NSAIDs, paracetamol has minimal anti-inflammatory activity in peripheral tissues and does not inhibit platelet function, making it safer for post-operative use. DOSAGE AND ADMINISTRATION

•      Adults: 300 to 600mg by slow intravenous injection or 1g (three to four ampoules) by IV infusion every 4 to 6 hours; maximum 4g per day from all sources

•      Children: dose based on weight, typically 15mg/kg every 4 to 6 hours

•      Administer by trained healthcare professionals in a clinical setting

SIDE EFFECTS

Common:

•      Hypotension with rapid intravenous administration (infuse slowly)

Warfarin anticoagulant effect may be enhanced with regular high-dose paracetamol; monitor INR. Probenecid reduces paracetamol glucuronidation, increasing plasma levels. Cholestyramine reduces paracetamol absorption if given within 1 hour.
