INDICATION

Bio-Fol Inj (Propofol) is an intravenous anaesthetic agent indicated for the induction and maintenance of general anaesthesia in adults and children above 3 years, and for conscious sedation in adults undergoing diagnostic or therapeutic procedures. It is also used for sedation of ventilated patients in intensive care units and for sedation during regional anaesthetic techniques.

MECHANISM OF ACTION

Propofol (2,6-diisopropylphenol) is a short-acting sedative-hypnotic agent with a mechanism of action primarily mediated through potentiation of GABA-A receptor chloride ion channel activity. It enhances the inhibitory effect of GABA at the GABA-A receptor, increasing chloride conductance across neuronal membranes, causing neuronal hyperpolarization and CNS depression. Propofol has a rapid onset (within 30 to 60 seconds of IV injection) and a short duration of action (5 to 10 minutes for induction doses) due to rapid redistribution from the CNS to peripheral tissues and fast hepatic metabolism, allowing precise control of anaesthetic depth and rapid recovery.

DOSAGE AND ADMINISTRATION

•      Induction of anaesthesia in adults: 1.5 to 2.5mg/kg by slow intravenous injection (approximately 40mg every 10 seconds)

•      Maintenance of anaesthesia: 4 to 12mg/kg/hour by continuous infusion

•      ICU sedation: 0.3 to 4mg/kg/hour titrated to desired level

•      Administered only by trained anesthetists with full resuscitation facilities available; not for patient self-administration

SIDE EFFECTS

Common:

•      Pain at the injection site (particularly with injection into smaller veins; use the antecubital fossa)

•      Hypotension during induction (monitor blood pressure closely)

•      Transient apnea during induction (have airway management ready)

•      Bradycardia

Less common:

•      Propofol infusion syndrome (PRIS) with prolonged high-dose infusion: metabolic acidosis, rhabdomyolysis, cardiac failure (rare but fatal; monitor closely in ICU patients)

•      Involuntary movements on induction

•      Anaphylaxis (rare; propofol emulsion contains soy and egg lecithin)

•      Green discoloration of urine (benign)

NOT RECOMMENDED IN

•      Known hypersensitivity to propofol or any excipient (egg lecithin, soya oil)

•      Use without full airway management and resuscitation facilities immediately available

•      Not for home use or patient self-administration

•      Caution in patients with known allergy to eggs or soya products

DRUG INTERACTIONS

CNS depressants including opioids, benzodiazepines, and inhalational an aesthetics potentiate propofol's anaesthetic effects; reduce doses accordingly. Fentanyl and alfentanil may increase propofol plasma levels. Rifampicin increases propofol clearance, reducing its duration. Valproate inhibits propofol glucuronidation, potentially increasing levels. Avoid mixing propofol with other drugs in the same syringe without confirmed compatibility.
