INDICATION

Yukon 10mg Tab is a non-selective NSAID indicated for the short-term management of moderate to severe acute pain in adults, including post-operative pain following surgical procedures, acute musculoskeletal pain, renal colic, dental pain, and other conditions requiring potent non-opioid analgesia. Ketorolac is one of the most potent oral NSAIDs available and is used when other analgesics, including paracetamol and weaker NSAIDs, are insufficient. Oral ketorolac is typically used as a step-down continuation following an initial injectable dose and is strictly limited to short-term use of no more than 5 days in total.

MECHANISM OF ACTION

Ketorolac tromethamine, a pyrrolizine carboxylic acid derivative, non-selectively inhibits COX-1 and COX-2 enzymes, blocking prostaglandin synthesis. By reducing prostaglandins, it diminishes pain, inflammation, and fever. Its analgesic potency is comparable to opioids; for example, 30 mg intramuscular ketorolac equals roughly 12 mg morphine for postoperative pain. This makes it highly effective for acute severe pain while avoiding opioid-related risks such as respiratory depression, dependence, or sedation.

DOSAGE AND ADMINISTRATION

•      Adults under 65 years: 10mg orally every 4 to 6 hours as needed; maximum 40mg per day.

•      Adults over 65 years, patients with renal impairment, or those under 50kg body weight: 10mg every 6 to 8 hours; maximum 30mg per day.

•      Total combined oral and injectable duration must not exceed 5 days.

•      Take with food or milk to reduce gastrointestinal irritation.

•      Do not exceed the maximum daily dose or extend beyond 5 days of use under any circumstances.

 

SIDE EFFECTS

•      Common: Nausea, dyspepsia, abdominal pain, diarrhea, headache, dizziness, drowsiness.

•      Less common: Gastrointestinal ulceration and bleeding, edema, elevated blood pressure, and elevated liver enzymes.

NOT RECOMMENDED IN

Yukon 10mg Tab is contraindicated in patients with active or a history of peptic ulcer disease, gastrointestinal bleeding, or perforation. It must not be used in patients with severe renal impairment (creatinine above 170 micromol/L), dehydration, confirmed cerebrovascular bleeding, hemorrhagic diathesis, or incomplete hemostasis. Contraindicated as prophylactic analgesia before major surgery, during labor or delivery, and in patients with known hypersensitivity to ketorolac, aspirin, or other NSAIDs. Not for use in children. Concomitant use with other NSAIDs or anticoagulants is contraindicated.

DRUG INTERACTIONS

Co-administration with anticoagulants, including warfarin and heparin, significantly increases serious bleeding risk and is contraindicated. Ketorolac combined with aspirin or other NSAIDs dramatically increases the risk of gastrointestinal ulceration and bleeding. Lithium plasma levels are increased by ketorolac through reduced renal clearance, increasing the risk of lithium toxicity. ACE inhibitors and ARBs may have their antihypertensive effects blunted. Diuretics may have their natriuretic efficacy reduced. Probenecid increases ketorolac plasma concentrations by competing for renal tubular secretion.
