INDICATION

Xiga Met 5/500mg Tab is a fixed-dose combination SGLT2 inhibitor and biguanide indicated for the management of type 2 diabetes mellitus in adults who are inadequately controlled on metformin alone or on dapagliflozin and metformin as separate tablets.

MECHANISM OF ACTION

Dapagliflozin selectively inhibits SGLT2 in the renal proximal tubule, blocking glucose reabsorption and increasing urinary glucose excretion. This lowers blood glucose independently of insulin, with associated weight loss, blood pressure reduction, and cardiovascular and renal protective effects. Metformin reduces hepatic glucose production and improves insulin sensitivity. The combination provides complementary mechanisms of glycaemic control with cardiovascular and renal organ protection from the dapagliflozin component.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet twice daily with meals

•      Assess eGFR before starting and monitor regularly

•      Swallow whole with water

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Genital mycotic infections (dapagliflozin-related)

•      Urinary tract infections

•      Nausea and diarrhoea (metformin-related)

•      Polyuria

Less common:

•      Diabetic ketoacidosis (euglycemic DKA; rare)

•      Volume depletion and hypotension

•      Lactic acidosis (rare; metformin in renal impairment)

•      Fournier's gangrene (rare; necrotizing fasciitis of the perineum; seek immediate medical attention if perineal pain or swelling develops)

NOT RECOMMENDED IN

•      eGFR below 45 ml/min/1.73m2

•      Type 1 diabetes or diabetic ketoacidosis

•      Recurrent genitourinary infections

•      Withhold before major surgery or iodinated contrast procedures

DRUG INTERACTIONS

Diuretics combined with SGLT2 inhibitors increase volume depletion and hypotension risk. Iodinated contrast: stop metformin before procedure. Insulin and sulfonylureas: Hypoglycaemia risk; dose reduction may be needed. Alcohol: lactic acidosis risk with metformin.
