INDICATION

Xenglu 25mg Tab is an oral SGLT-2 inhibitor indicated as an adjunct to diet and exercise for the management of blood glucose in adults with type 2 diabetes mellitus. It can be used as monotherapy when metformin is not tolerated, or as add-on combination therapy with other glucose-lowering agents including insulin, when existing treatment fails to achieve adequate glycaemic control. MECHANISM OF ACTION

Xenglu 25mg contains empagliflozin, an SGLT-2 inhibitor that acts directly on the kidneys to lower blood sugar. Under normal conditions, the SGLT-2 protein in kidney tubules reabsorbs approximately 90% of filtered glucose back into the bloodstream. Empagliflozin selectively blocks this transporter, preventing glucose reabsorption and causing excess sugar to be excreted through urine. This insulin-independent mechanism lowers blood glucose without stimulating the pancreas, making it effective even in patients with impaired insulin secretion. The reduction in plasma glucose, blood pressure, and body weight, combined with reduced cardiac preload and afterload, contributes to the cardiovascular and renal benefits observed with this drug class.

 

DOSAGE AND ADMINISTRATION

•      Starting dose: 10 mg once daily, taken with or without food

•      If additional glycaemic control is needed and the 10 mg dose is well-tolerated, increase to 25 mg once daily

•      Not recommended for use in children under 18 years of age

 

SIDE EFFECTS

Common: Urinary tract infections, genital yeast infections in men and women, increased urination, nasopharyngitis.

Less common: Low blood pressure and dizziness on standing, hypoglycaemia when used with sulphonylureas or insulin, elevated serum creatinine, rare diabetic ketoacidosis

 

NOT RECOMMENDED IN

Xenglu 25mg is contraindicated in patients with type 1 diabetes mellitus and must not be used for the treatment of diabetic ketoacidosis, as it provides no clinical benefit in these settings and may worsen outcomes. It is not suitable for patients with severe renal impairment (eGFR below 30 mL/min/1.73m²), end-stage renal disease, or those receiving dialysis. Patients with a known hypersensitivity to empagliflozin or any excipient in the formulation should not use this product. Use during pregnancy is not recommended due to risk to the developing fetus, and breastfeeding should be avoided during treatment. Patients with recurrent genital or urinary tract infections should discuss individual risk with their physician before initiation.

 

DRUG INTERACTIONS

Empagliflozin used alongside insulin or insulin secretagogues such as sulphonylureas increases the risk of hypoglycaemia; the dose of the sulphonylurea or insulin should be reduced when initiating Xenglu. Diuretics taken concurrently can exacerbate volume depletion, increasing the risk of dehydration, low blood pressure, and acute kidney injury, particularly in elderly patients. ACE inhibitors and angiotensin receptor blockers used together with Xenglu may further affect renal haemodynamics and electrolyte balance, requiring monitoring. Alcohol consumption raises the risk of both hypoglycaemia and euglycaemic ketoacidosis and should be limited. Inform the prescribing physician of all medications, supplements, and herbal products before starting Xenglu 25mg.
