INDICATION

Votadol 50mg Cap is a centrally acting opioid analgesic indicated for the treatment of moderate to moderately severe acute and chronic pain. It is used in post-operative pain, musculoskeletal pain, neuropathic pain, cancer pain, and other conditions where non-opioid analgesics provide insufficient pain relief.

MECHANISM OF ACTION

Tramadol has a dual mechanism of action. It is a weak mu-opioid receptor agonist, providing analgesic activity through opioid pathways, including reduction of pain transmission in the spinal cord and brain. In addition, tramadol inhibits the neuronal reuptake of both serotonin and norepinephrine, enhancing descending inhibitory pain pathways independently of opioid receptors. This noradrenergic and serotonergic component contributes significantly to its analgesic effect, particularly in neuropathic pain. The active metabolite O-desmethyltramadol (M1), formed via CYP2D6, has approximately 200 times higher mu-opioid receptor affinity than the parent compound and is responsible for a substantial portion of the opioid analgesia.

DOSAGE AND ADMINISTRATION

•      Adults: 50mg to 100mg every 4 to 6 hours as needed; maximum 400mg per day

•      Elderly patients and those with renal or hepatic impairment: start at 50mg and extend dosing intervals

•      Take with or without food; swallow whole with water

•      Always follow the dosage prescribed by your doctor; use the lowest effective dose for the shortest duration

SIDE EFFECTS

Common:

•      Nausea and vomiting (most common, particularly on initiation)

•      Dizziness and somnolence

•      Constipation

•      Headache and dry mouth

Less common:

•      Serotonin syndrome when combined with other serotonergic drugs (potentially serious)

•      Severe renal impairment (CrCl below 10 ml/min) or severe hepatic impairment

•      Known hypersensitivity to tramadol or opioids; epilepsy not adequately controlled; pregnancy and breastfeeding

DRUG INTERACTIONS

MAO inhibitors are contraindicated within 14 days of tramadol use due to the risk of life-threatening serotonin syndrome and seizures. Concurrent serotonergic drugs including SSRIs, SNRIs, triptans, linezolid, and St John's Wort significantly increase serotonin syndrome risk. CYP2D6 inhibitors such as fluoxetine and paroxetine reduce conversion of tramadol to its active M1 metabolite, potentially reducing analgesic efficacy while increasing serotonin-related side effects from the parent compound. CNS depressants including opioids, benzodiazepines, and alcohol produce additive respiratory depression and sedation; avoid combination. Carbamazepine induces tramadol metabolism, reducing plasma levels and analgesic efficacy.
