INDICATION

Vorimont 200mg Tab is a second-generation triazole antifungal indicated for the treatment of serious and invasive fungal infections in adults and children aged 2 years and older. Its primary approved indications include invasive aspergillosis, candidemia in non-neutropenic patients, disseminated Candida infections affecting the skin, abdomen, kidneys, bladder, and wounds, and esophageal candidiasis. Vorimont is also indicated for serious fungal infections caused by Scedosporium apiospermum and Fusarium species, particularly in patients who are intolerant of, or refractory to, other antifungal therapies. Given its broad-spectrum activity against clinically relevant fungi, it is frequently used in immunocompromised patients, transplant recipients, and those undergoing chemotherapy.

 

MECHANISM OF ACTION

Vorimont contains voriconazole, a triazole antifungal that works by blocking a key fungal enzyme called lanosterol 14-alpha-demethylase. This enzyme is essential for converting lanosterol into ergosterol, the structural component that keeps fungal cell membranes intact and functional. When voriconazole inhibits this enzyme, ergosterol production halts, and toxic sterol intermediates build up within the cell membrane. The resulting membrane becomes structurally compromised, leaky, and unable to maintain normal cellular processes. Fungal cells lose their integrity and die, effectively clearing the infection from the body over the course of treatment.

 

DOSAGE AND ADMINISTRATION

•      Adults: 400 mg every 12 hours for the first 24 hours; maintenance: 200 mg every 12 hours

•      Adults: 200 mg every 12 hours for the first 24 hours; maintenance: 100 mg every 12 hours

•      If response is inadequate, maintenance dose may be increased to 300 mg every 12 hours (≥40 kg) or 150 mg every 12 hours (<40 kg)

•      Take on an empty stomach at least 1 hour before or 1 hour after a meal for optimal absorption.

•      Take every 12 hours at the same time each day to maintain consistent drug levels.

SIDE EFFECTS

Common: Visual disturbances, nausea, vomiting, diarrhoea, abdominal pain, headache, fever, rash, elevated liver enzymes.

Less common: Peripheral oedema, hallucinations, confusion, QT prolongation, serious skin reactions

 

NOT RECOMMENDED IN

Vorimont is contraindicated in patients with a known hypersensitivity to voriconazole or any other triazole antifungal agent. It must not be used during pregnancy (Pregnancy Category D) or breastfeeding due to a significant risk of fetal harm and unknown excretion in breast milk. Co-administration with rifampicin, carbamazepine, phenobarbitone, long-acting barbiturates, St. John's Wort, high-dose ritonavir (400 mg twice daily), and naloxegol is contraindicated due to substantial reduction of voriconazole plasma concentrations. Patients with a pre-existing history of QT prolongation or those receiving other QT-prolonging medications should be managed with caution under specialist supervision.

 

DRUG INTERACTIONS

Voriconazole potently inhibits CYP2C9, CYP2C19, and CYP3A4 and significantly raises the plasma levels of numerous co-administered drugs. Tacrolimus, cyclosporine, and sirolimus levels increase markedly and require dose reduction and therapeutic drug monitoring. Warfarin anticoagulation is enhanced, requiring close INR monitoring. Phenytoin levels are reduced by voriconazole, yet phenytoin simultaneously lowers voriconazole levels, requiring reciprocal dose adjustments. Sulphonylureas, statins, benzodiazepines, and calcium channel blockers all accumulate at higher plasma concentrations when combined with voriconazole. Efavirenz may be co-administered only if voriconazole maintenance is increased to 400 mg every 12 hours and efavirenz is reduced to 300 mg once daily. Always disclose all current medications to the prescribing physician before starting Vorimont.
