INDICATION

Vanzak 20mg Tab is indicated for the management of acid-related gastrointestinal disorders in adults, including erosive esophagitis due to gastroesophageal reflux disease (GERD), non-erosive reflux disease, gastric and duodenal ulcers, and Helicobacter pylori eradication when used with appropriate antibiotics. As a potassium-competitive acid blocker (P-CAB), vonoprazan provides rapid, potent, and sustained gastric acid suppression, making it particularly valuable in patients with inadequate response to proton pump inhibitors (PPIs).

MECHANISM OF ACTION

Vonoprazan competitively and reversibly inhibits the potassium-binding site of the H⁺/K⁺-ATPase enzyme (proton pump) in gastric parietal cells. Unlike conventional PPIs, it does not require acid activation and remains active at neutral pH, inhibiting both active and resting proton pumps. This results in immediate onset of action and sustained 24-hour acid suppression, including effective nocturnal control and reduced variability in therapeutic response.

DOSAGE AND ADMINISTRATION

●     Erosive esophagitis: 20mg once daily for 4–8 weeks.

●     H. pylori eradication: 20mg twice daily for 7 days in combination with antibiotics.

●     Maintenance therapy: 10–20mg once daily based on clinical response.

●     Take at the same time each day to maintain consistent acid suppression.

 

SIDE EFFECTS

●     Common: Diarrhea, nausea, abdominal discomfort, constipation, headache.

●     Less common: Elevated liver enzymes, flatulence, dizziness, dry mouth, skin rash.

NOT RECOMMENDED IN

Contraindicated in patients with hypersensitivity to vonoprazan. Use with caution in severe hepatic impairment due to CYP3A4 metabolism. Therapy should not replace proper evaluation of alarm symptoms such as unexplained weight loss, persistent dysphagia, hematemesis, or melena. Safety in pregnancy, breastfeeding, and pediatric populations is not well established.

DRUG INTERACTIONS

Primarily metabolized via CYP3A4. Strong inhibitors (e.g., clarithromycin) may increase plasma levels, while inducers (e.g., rifampicin) may reduce efficacy. May impair absorption of drugs requiring acidic pH such as ketoconazole and iron salts. Avoid concurrent use with atazanavir and nelfinavir due to reduced antiviral absorption.
