INDICATION
Valsar-M 5/80mg Tab is a fixed-dose combination antihypertensive prescribed for the management of essential hypertension in adults whose blood pressure is not adequately controlled on either amlodipine or valsartan monotherapy. It is also used in patients for whom combination therapy is considered more appropriate from the outset. By combining a calcium channel blocker with an angiotensin II receptor blocker, Valsar-M delivers superior blood pressure lowering through two complementary mechanisms, reducing the risk of heart attack, stroke, and kidney disease in hypertensive patients.
MECHANISM OF ACTION
Valsar-M works through two distinct but complementary blood pressure-lowering pathways. Amlodipine is a dihydropyridine calcium channel blocker that prevents calcium from entering the smooth muscle cells of blood vessel walls. Without calcium influx, these cells cannot contract, causing blood vessels to relax and widen, reducing peripheral vascular resistance and blood pressure. Valsartan is an angiotensin II receptor blocker (ARB) that prevents angiotensin II, a potent vasoconstrictor hormone, from binding to its AT1 receptors. By blocking this receptor, valsartan prevents blood vessel constriction, reduces aldosterone-driven fluid retention, and lowers blood pressure effectively. Together, they produce greater antihypertensive efficacy than either agent alone.
DOSAGE AND ADMINISTRATION
ā¢Ā Ā Ā Ā Ā Usual dose: One tablet (5mg/80mg) once daily, taken orally with or without food.

SIDE EFFECTS
ā¢Ā Ā Ā Ā Ā Common: Peripheral edema (ankle swelling from amlodipine), dizziness, headache, nasopharyngitis, flushing.
ā¢Ā Ā Ā Ā Ā Less common: Fatigue, nausea, hyperkalemia, renal impairment, cough (less common than with ACE inhibitors).
NOT RECOMMENDED IN
Valsar-M 5/80mg Tab is contraindicated in patients with known hypersensitivity to amlodipine, valsartan, or other dihydropyridine calcium channel blockers. It is absolutely contraindicated during pregnancy, as valsartan can cause serious fetal harm. Not recommended for patients with severe hepatic impairment or biliary cirrhosis. Concurrent use with aliskiren in patients with diabetes or renal impairment is contraindicated. Patients with severe aortic stenosis should use amlodipine with caution.
DRUG INTERACTIONS
Valsartan taken with potassium-sparing diuretics, potassium supplements, or other medicines that increase potassium levels can lead to high potassium (hyperkalemia), so regular monitoring is important. NSAIDs may reduce the blood pressureālowering effect and increase the risk of kidney problems. Medicines like ketoconazole and erythromycin can increase amlodipine levels in the body, while rifampicin can reduce its effectiveness. Using it with other blood pressure medicines or vasodilators can further lower blood pressure and may cause dizziness or orthostatic hypotension. The risk of lithium toxicity may also increase when used with ARBs.
