INDICATION
Vadil 6.25mg Tab is a non-selective beta-blocker with alpha-1 blocking activity, prescribed for the management of mild to severe chronic heart failure in combination with standard therapy, hypertension, and left ventricular dysfunction following a myocardial infarction. It is also indicated for stable angina pectoris. The 6.25mg dose is the standard initiating dose for heart failure and is titrated upward gradually as tolerated.
MECHANISM OF ACTION
Carvedilol exerts its therapeutic effects through multiple pharmacological mechanisms. As a non-selective beta-blocker, it blocks both beta-1 receptors in the heart and beta-2 receptors in the lungs and blood vessels, reducing heart rate, cardiac workload, and oxygen demand. Its alpha-1 blocking activity causes vasodilation, lowering peripheral vascular resistance and blood pressure without reflex tachycardia. In heart failure, this dual blockade reduces the harmful effects of chronic sympathetic nervous system overactivation on the heart.
DOSAGE AND ADMINISTRATION
ā¢Ā Ā Ā Ā Ā Heart failure: Starting dose 3.125mg twice daily for two weeks; double the dose every two weeks to 6.25mg, 12.5mg, and then 25mg twice daily as tolerated.
ā¢Ā Ā Ā Ā Ā Hypertension: 6.25mg twice daily, increasing to 12.5mg or 25mg twice daily as needed.
ā¢Ā Ā Ā Ā Ā Never stop abruptly; taper gradually over one to two weeks under physician supervision.
SIDE EFFECTS
ā¢Ā Ā Ā Ā Ā Common: Dizziness, fatigue, hypotension, bradycardia, headache, nausea, weight gain.

NOT RECOMMENDED IN
Vadil 6.25mg Tab is contraindicated in patients with decompensated heart failure requiring intravenous inotropic therapy, severe bradycardia, second or third-degree AV block without a pacemaker, or cardiogenic shock. It must not be used in patients with significant hepatic impairment. It is not recommended in those with severe reactive airway disease (asthma) due to beta-2 blockade. Abrupt withdrawal can precipitate angina or myocardial infarction; always taper. Use during pregnancy requires physician evaluation.
DRUG INTERACTIONS
Concurrent use with diltiazem or verapamil can cause severe bradycardia and AV block; avoid this combination or monitor closely. Carvedilol significantly increases digoxin plasma concentrations; regular monitoring of digoxin levels is required. Antidiabetic medications, including insulin, may have masked hypoglycemia symptoms due to beta-blockade; glucose monitoring is essential. Rifampicin significantly reduces carvedilol levels. Cyclosporine levels may increase with carvedilol co-administration. MAOIs and other vasodilators may enhance blood pressure-lowering effects. All medications should be reviewed before initiating carvedilol therapy.
