INDICATION

Utramex 500mg Caps is an antifibrinolytic agent indicated for the prevention and treatment of hemorrhage associated with excessive fibrinolysis. It is used in menorrhagia (heavy menstrual bleeding), post-traumatic bleeding, surgical bleeding including dental extraction in patients on anticoagulants, hematuria, and epistaxis. It is also used in hemophilia patients undergoing surgical procedures.

MECHANISM OF ACTION

Tranexamic acid is a synthetic derivative of the amino acid lysine that competitively inhibits the activation of plasminogen to plasmin by blocking the lysine-binding sites on plasminogen molecules. Plasmin is the enzyme responsible for dissolving fibrin blood clots. By preventing plasminogen activation, tranexamic acid preserves fibrin clots at sites of bleeding and reduces excessive fibrinolysis. It does not promote clot formation but rather prevents the premature breakdown of clots that have already formed, thereby reducing bleeding duration and blood loss.

DOSAGE AND ADMINISTRATION

•      Menorrhagia: 1g (two 500mg capsules) three times daily for up to 4 days during menstruation

•      Surgical prophylaxis: 1g two to three times daily starting 1 day before and continuing for 1 to 2 days after procedure

•      Epistaxis and hematuria: 1g three times daily for 7 days

•      Always follow the dosage prescribed by your doctor; dose reduction required in renal impairment

SIDE EFFECTS

Common:

•      Nausea, vomiting, and diarrhoea

•      Abdominal discomfort

•      Headache and dizziness

•      Color vision disturbances with prolonged use (rare at standard doses; ophthalmological monitoring recommended for long-term therapy)

Less common:

•      Thromboembolic events: deep vein thrombosis, pulmonary embolism (risk particularly in patients with pre-existing thrombophilia)

•      Hypersensitivity reactions including rash

•      Hypotension with rapid intravenous injection (not applicable at oral doses)

•      Seizures at high doses (particularly in cardiac surgery settings; not applicable at standard oral doses)

NOT RECOMMENDED IN

•      Active thromboembolic disease including deep vein thrombosis or pulmonary embolism

•      History of convulsions (high-dose intravenous use is associated with seizure risk)

•      Severe renal impairment without dose reduction (tranexamic acid accumulates)

•      Known hypersensitivity to tranexamic acid; concurrent use with hormonal contraceptives increases thrombosis risk

DRUG INTERACTIONS

Hormonal contraceptives combined with tranexamic acid increase the risk of thromboembolic events; use with caution and advise patients of the increased risk. Factor IX complex concentrates and anti-inhibitor coagulant concentrates should not be combined with tranexamic acid due to increased risk of thrombosis. Retinoids (isotretinoin, acitretin) combined with tranexamic acid may theoretically increase the risk of thromboembolism in susceptible patients. Tranexamic acid does not have significant pharmacokinetic interactions with common cardiovascular or analgesic medications at standard oral doses.
