INDICATION

Treviamet 50mg+1000mg Tab is a fixed-dose combination DPP-4 inhibitor and biguanide indicated for the management of type 2 diabetes mellitus in adults inadequately controlled on metformin 1000mg alone, or on sitagliptin and metformin as separate tablets.

MECHANISM OF ACTION

Sitagliptin inhibits DPP-4, increasing active GLP-1 and GIP incretin levels, stimulating glucose-dependent insulin secretion and suppressing glucagon. Metformin reduces hepatic glucose production through AMPK activation, improves peripheral insulin sensitivity, and reduces intestinal glucose absorption. The combination addresses complementary pathophysiological defects in type 2 diabetes with additive glycaemic benefit, without causing weight gain or hypoglycemia as monotherapy.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet twice daily with meals

•      Monitor renal function at baseline and periodically

•      Swallow whole with a full glass of water

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea (metformin-related)

•      Nasopharyngitis (sitagliptin-related)

•      Headache

•      Abdominal discomfort

Less common:

•      Lactic acidosis (rare; risk in renal impairment)

•      Pancreatitis (rare)

•      Vitamin B12 deficiency with long-term metformin

•      Hypoglycaemia with insulin or sulfonylurea

DRUG INTERACTIONS

Iodinated contrast media: stop metformin before procedure and restart 48 hours after if renal function is stable. Alcohol: avoid excessive intake. Cationic drugs reduce metformin renal clearance. Insulin and sulfonylureas: hypoglycemia risk. ACE inhibitors may enhance sitagliptin effect.
