INDICATION

Tramapar Tab is a dual-action analgesic combination prescribed for the management of moderate to severe pain that is not adequately controlled by non-opioid analgesics alone. It is indicated for acute pain following surgery or dental procedures, chronic musculoskeletal pain, lower back pain, pain associated with osteoarthritis, migraine, and other pain conditions requiring both central and peripheral analgesic activity. MECHANISM OF ACTION

Tramapar combines two analgesics with different but complementary pain-relieving mechanisms. Tramadol acts centrally in the brain and spinal cord by binding weakly to mu-opioid receptors and simultaneously inhibiting the reuptake of serotonin and norepinephrine, enhancing the body's natural pain suppression pathways. Paracetamol acts peripherally and centrally by inhibiting prostaglandin synthesis in the central nervous system and blocking pain signal transmission at the spinal level, while also reducing fever. Together, they produce a synergistic analgesic effect that targets pain through two independent pathways, making the combination more effective than either drug alone at lower individual doses.

DOSAGE AND ADMINISTRATION

•      Adults: One tablet every four to six hours as needed for pain relief.

•      Maximum dose: Four tablets per day (equivalent to 1300mg paracetamol and 150mg tramadol).

DRUG INTERACTIONS

Co-administration with MAOIs is strictly contraindicated, as it can cause serotonin syndrome. Concurrent use with CNS depressants including benzodiazepines, antidepressants, antipsychotics, sedatives, and alcohol significantly increases the risk of sedation and respiratory depression. Tramadol with SSRIs or SNRIs further raises the risk of serotonin syndrome. Warfarin and other anticoagulants may have enhanced effects when combined with paracetamol; INR monitoring is advised. CYP3A4 inducers like rifampicin can reduce tramadol efficacy, while inhibitors can increase its plasma levels.
