INDICATION

Topival 25mg Tab is a broad-spectrum anticonvulsant indicated as monotherapy or adjunctive therapy for partial-onset seizures with or without secondary generalization, and for primary generalized tonic-clonic seizures in adults and children. It is also indicated for the prevention of migraine headaches in adults.

MECHANISM OF ACTION

Topiramate has multiple complementary mechanisms. It blocks voltage-gated sodium channels, preventing the rapid repetitive firing of neurons that generates seizures. It enhances GABA-A receptor activity, increasing inhibitory neurotransmission. It antagonizes AMPA and kainite subtypes of glutamate receptors, reducing excitatory neurotransmission. It also weakly inhibits carbonic anhydrase, which may contribute to its anticonvulsant activity. The combination of these mechanisms provides broad-spectrum anticonvulsant activity. The 25mg dose is the standard starting and titration dose, taken once or twice daily.

DOSAGE AND ADMINISTRATION

•      Epilepsy (monotherapy): start 25mg once daily at night; increase by 25 to 50mg per week to 200 to 400mg/day in divided doses

•      Migraine prophylaxis: 25mg once daily at night; increase weekly to 50mg twice daily

•      Adjunctive therapy: titrate more slowly from 25mg per day

•      Always follow the titration schedule prescribed by your neurologist; never stop abruptly

SIDE EFFECTS

Common:

•      Somnolence and cognitive slowing (word-finding difficulties, concentration problems)

•      Dizziness and ataxia

•      Paresthesia (tingling in extremities; common and usually benign)

•      Anorexia and weight loss

Less common:

•      Metabolic acidosis with elevated blood ammonia

•      Kidney stones (drink plenty of water to minimize risk)

•      Acute myopia and secondary angle-closure glaucoma (seek immediate attention if sudden vision change or eye pain)

•      Hyperthermia and reduced sweating, especially in children; monitor in hot weather

NOT RECOMMENDED IN

•      Known hypersensitivity to topiramate

•      Metabolic acidosis with concurrent metformin (may worsen acidosis)

•      Pregnancy: associated with increased risk of cleft lip and palate; use reliable contraception

•      Avoid combined oral contraceptives at doses above 200mg/day (reduces contraceptive efficacy)

DRUG INTERACTIONS

Topiramate reduces the efficacy of estrogen-containing oral contraceptives at doses above 200mg/day; alternative contraception is required. Valproic acid combined with topiramate increases the risk of hyperammonemia and encephalopathy; monitor ammonia levels. CNS depressants and alcohol add to cognitive impairment and sedation. Carbonic anhydrase inhibitors such as acetazolamide combined with topiramate increase kidney stone risk. Phenytoin and carbamazepine reduce topiramate plasma levels.
