INDICATION

Tocinox Inj is a synthetic oxytocin indicated for the induction and augmentation of labor at term, control of postpartum hemorrhage and uterine atony, management of incomplete or inevitable miscarriage, and acceleration of placental delivery. It is used only in a hospital setting under obstetric supervision.

MECHANISM OF ACTION

Oxytocin is a synthetic nonapeptide identical to the endogenous posterior pituitary hormone. It acts on specific oxytocin receptors in the myometrium, which are G-protein coupled receptors that increase intracellular calcium and activate phospholipase C, leading to enhanced actin-myosin interaction and uterine smooth muscle contraction. Uterine sensitivity to oxytocin increases progressively with gestational age, peaking at term. In postpartum hemorrhage, oxytocin-induced myometrial contractions compress uterine blood vessels, reducing blood loss and promoting hemostasis. Oxytocin also stimulates myoepithelial cells in mammary glands, promoting milk ejection during lactation.

DOSAGE AND ADMINISTRATION

•      Labor induction: 1 to 2 mud/minute by IV infusion, increasing by 1 to 2 mU/minute every 30 minutes until regular contractions established; maximum 20 mU/minute

•      Postpartum hemorrhage: 10 IU IM immediately after delivery of the baby, or 5 to 10 IU by slow IV injection

•      Always administered by trained obstetric healthcare professionals with continuous fetal monitoring

•      Dosage titrated by the obstetrician based on uterine response and fetal wellbeing

SIDE EFFECTS

Common:

•      Uterine hyperstimulation (contractions too frequent or prolonged; reduce or stop infusion)

•      Nausea and vomiting

•      Headache

•      Fetal heart rate abnormalities during labor induction

Less common:

•      Severe hypertension or hypotension with rapid IV bolus injection

•      Water intoxication and hyponatremia with prolonged high-dose infusion (monitor fluid balance)

•      Uterine rupture in susceptible patients (prior uterine scar, overdosage)

•      Anaphylaxis (rare)

NOT RECOMMENDED IN

•      Hypertonic uterine contractions or fetal distress

•      Placenta previa or vasa previa

•      Pelvic disproportion or obstructed labor

•      Known hypersensitivity to oxytocin; do not administer as an IV bolus for induction (IV infusion only)

DRUG INTERACTIONS

Prostaglandins used concurrently with oxytocin produce additive uterotonic effects and increase the risk of uterine hyperstimulation; do not administer simultaneously. Inhalational anaesthetic agents such as halothane reduce uterine responsiveness to oxytocin and may cause hypotension. Vasoconstrictors combined with oxytocin can cause severe hypertension. Epidural anaesthesia combined with oxytocin may produce hypotension; monitor blood pressure closely.
