INDICATION

Tigasor 25mg Cap is a systemic retinoid indicated for the treatment of severe psoriasis that has not responded adequately to topical therapy or phototherapy. It is also indicated for other severe keratinization disorders including lamellar ichthyosis, Darier's disease, and palmoplantar keratoderma.

MECHANISM OF ACTION

Acitretin is the active metabolite of etretinate and a second-generation aromatic retinoid that binds to retinoic acid receptors (RARs) in the nucleus, modulating the transcription of genes involved in epidermal proliferation, differentiation, and inflammatory responses. In psoriasis, it normalizes the abnormally rapid keratinocyte proliferation and restores normal epidermal differentiation, reducing plaque formation, scale, and skin thickening. Acitretin also reduces inflammatory infiltration in the dermis and modulates immune cell activity in psoriatic skin. It is often used in combination with phototherapy for enhanced efficacy.

DOSAGE AND ADMINISTRATION

•      Initial dose: 25mg once daily with the main meal

•      Dose may be adjusted between 25mg and 50mg per day based on response and tolerability

•      Treatment course typically 3 to 6 months; reassess regularly

•      Always follow the dosage and monitoring schedule prescribed by your dermatologist

SIDE EFFECTS

Common:

•      Dry lips (cheilitis), dry skin, dry eyes, and dry nasal mucosa

•      Elevated triglycerides and cholesterol (monitor lipids monthly initially)

•      Alopecia (hair loss, usually reversible after stopping)

•      Skin fragility and increased photosensitivity

Less common:

•      Teratogenicity: causes severe fetal malformations; pregnancy must be absolutely prevented during treatment and for 3 years after stopping in females

•      Elevated liver enzymes and hepatotoxicity (monitor LFTs regularly)

•      Night blindness and visual disturbances

•      Musculoskeletal pain and premature epiphyseal closure in children

•      Pregnancy or planned pregnancy within 3 years of stopping acitretin in females

•      Women of childbearing potential not using two reliable contraceptive methods simultaneously

•      Severe hepatic or renal impairment

•      Concurrent use with methotrexate or vitamin A supplements; concurrent tetracyclines

DRUG INTERACTIONS

Methotrexate combined with acitretin markedly increases hepatotoxicity risk; concurrent use is contraindicated. Vitamin A supplements increase the risk of hypervitaminosis A toxicity. Tetracycline antibiotics combined with retinoids increase intracranial hypertension risk; avoid. Alcohol must be completely avoided during treatment and for 2 months after stopping (acitretin is converted back to etretinate in the presence of alcohol, greatly extending teratogenic risk). Phenytoin protein binding may be reduced by acitretin.
