INDICATION

Terlip 1mg Inj is a synthetic vasopressin analogue indicated for the treatment of bleeding esophageal varices in patients with portal hypertension due to liver cirrhosis. It is also used in hepatorenal syndrome to improve renal perfusion and in septic shock refractory to norepinephrine.

MECHANISM OF ACTION

Terlipressin is a prodrug that is enzymatically converted to lysine-vasopressin in body tissues. Lysine-vasopressin acts on vasopressin V1 receptors on vascular smooth muscle, causing splanchnic and systemic vasoconstriction. In variceal bleeding, this reduces portal venous blood flow and portal pressure, controlling hemorrhage from esophageal varices. In hepatorenal syndrome, it reverses splanchnic vasodilation, improving effective arterial blood volume and renal perfusion. Its longer half-life than vasopressin allows intermittent bolus dosing rather than continuous infusion.

DOSAGE AND ADMINISTRATION

      Variceal bleeding: 1 to 2mg by slow intravenous injection every 4 to 6 hours until hemostasis, for up to 72 hours

      Hepatorenal syndrome: 0.5 to 1mg every 4 to 6 hours, titrated to response

      Administered by slow intravenous injection by trained healthcare professionals only

      Always prescribed and monitored by a specialist; not for self-administration

SIDE EFFECTS

Common:

      Abdominal cramps and diarrhoea

      Hypertension

      Pallor and sweating

      Headache

Less common:

      Cardiac arrhythmias and bradycardia

      Peripheral and mesenteric ischemia

      Respiratory failure (monitor oxygen saturation continuously)

      Hyponatremia with prolonged use

NOT RECOMMENDED IN

      Known hypersensitivity to Terlipressin or vasopressin analogues

      Pregnancy (causes uterine contractions and fetal harm)

      Septic shock with severe hypoxia

      Severe cardiovascular disease including unstable angina or recent myocardial infarction

DRUG INTERACTIONS

Terlipressin combined with beta-blockers and digoxin increases the risk of cardiac arrhythmias; monitor ECG during co-administration. NSAIDs may reduce the renal benefit in hepatorenal syndrome. Concurrent vasopressors require careful hemodynamic monitoring to avoid excessive hypertension. Albumin infusion is routinely co-administered with Terlipressin in hepatorenal syndrome to improve efficacy.
