INDICATION
Terlax 2mg Tab is a centrally acting alpha-2 adrenergic agonist used as a muscle relaxant for the management of spasticity associated with neurological conditions including multiple sclerosis, spinal cord injury, and stroke. It is also used for acute muscle spasm and painful musculoskeletal conditions.
MECHANISM OF ACTION
Tizanidine acts as a selective agonist at alpha-2 adrenergic receptors in the spinal cord and brainstem, enhancing presynaptic inhibition of motor neurons. By increasing inhibitory neurotransmission, it reduces the release of excitatory amino acids (glutamate and aspartate) from spinal interneurons, decreasing polysynaptic reflex activity and lowering skeletal muscle tone. This central mechanism reduces spasticity without the peripheral neuromuscular blockade of agents like baclofen, and with less sedation than diazepam at equivalent antispastic doses.
DOSAGE AND ADMINISTRATION
• Initial dose: 2mg once daily at night to minimize daytime sedation
• Increase by 2mg every 3 to 4 days based on response and tolerability
• Usual effective dose: 6 to 12mg per day in 3 divided doses; maximum 36mg per day
• Always follow the dose titration schedule prescribed by your neurologist; never stop abruptly
SIDE EFFECTS
Common:
• Somnolence and sedation (most common)
• Dizziness and dry mouth
• Hypotension (low blood pressure)
• Fatigue and weakness
Less common:
• Elevated liver enzymes and hepatotoxicity (monitor LFTs in first 6 months of therapy)
• Hallucinations and confusion
• Bradycardia
• Rebound hypertension and tachycardia on abrupt discontinuation
NOT RECOMMENDED IN
• Severe hepatic impairment (hepatotoxicity risk; monitor LFTs carefully)
• Concurrent use with fluvoxamine or ciprofloxacin (potent CYP1A2 inhibitors; markedly increase tizanidine exposure)
• Concurrent use with other antihypertensives or alpha-2 agonists (additive hypotension)
• Known hypersensitivity to tizanidine
DRUG INTERACTIONS
CYP1A2 inhibitors, particularly fluvoxamine and ciprofloxacin, dramatically increase tizanidine plasma levels, leading to severe hypotension, bradycardia, and sedation; these combinations are contraindicated. Oral contraceptives containing estrogen reduce CYP1A2 activity and increase tizanidine levels by approximately 50%; reduce tizanidine dose accordingly. CNS depressants including alcohol, benzodiazepines, and opioids add to sedation. Antihypertensives produce additive hypotensive effects.
