INDICATION

Tenormin 100mg Tab is a cardioselective beta-1 adrenergic blocker indicated for the treatment of hypertension, stable angina pectoris, and management of tachyarrhythmias. It is also used in the early and long-term management of acute myocardial infarction to reduce mortality and heart failure risk.

MECHANISM OF ACTION

Atenolol selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, myocardial contractility, and cardiac output, which collectively lower blood pressure and myocardial oxygen demand. Its beta-1 selectivity minimizes adverse effects on beta-2 receptors in the lungs and peripheral vasculature, making it safer than non-selective beta-blockers in patients with mild to moderate obstructive airways disease. Atenolol is hydrophilic and does not cross the blood-brain barrier significantly, reducing CNS side effects such as nightmares and depression.

DOSAGE AND ADMINISTRATION

•      Hypertension and angina: 50 to 100mg once daily

•      Higher doses up to 200mg may be used in arrhythmia management

•      Take at the same time each day with or without food

•      Never stop abruptly; taper the dose gradually under medical supervision

SIDE EFFECTS

Common:

•      Bradycardia (slow heart rate)

•      Fatigue and weakness

•      Cold extremities (peripheral vasoconstriction)

•      Dizziness

Less common:

•      Bronchospasm in asthmatic patients (use with great caution)

•      Erectile dysfunction

•      Depression (less than with lipophilic beta-blockers)

•      AV block in susceptible patients

NOT RECOMMENDED IN

•      Cardiogenic shock or decompensated heart failure requiring inotropic support

•      Second or third-degree AV block without a pacemaker

•      Severe bradycardia (below 50 beats per minute)

•      Known hypersensitivity to atenolol; uncontrolled asthma or severe COPD

DRUG INTERACTIONS

Verapamil and diltiazem combined with atenolol significantly increase the risk of AV block and bradycardia; avoid combination. Antihypertensives including diuretics and ACE inhibitors have additive blood pressure-lowering effects. Clonidine: if both are discontinued, taper atenolol first to prevent rebound hypertension. Beta-blockers mask hypoglycemia tachycardia in diabetic patients on insulin.
