INDICATION

Tempro 0.4mg Cap is a selective alpha-1A adrenergic receptor blocker indicated for the treatment of signs and symptoms of benign prostatic hyperplasia (BPH) in adult men. It improves urinary flow and relieves lower urinary tract symptoms including weak stream, hesitancy, urgency, and nocturia.

MECHANISM OF ACTION

Tamsulosin selectively blocks alpha-1A adrenergic receptors in the prostate gland, bladder neck, and urethra, causing smooth muscle relaxation and reducing urethral resistance without significantly lowering blood pressure. Its selectivity for prostatic alpha-1A receptors over vascular alpha-1B receptors is the basis for its favorable cardiovascular tolerability profile. The modified-release formulation provides consistent plasma levels over 24 hours, enabling once-daily dosing with reduced peak concentration-related side effects.

DOSAGE AND ADMINISTRATION

•      Adults: 0.4mg once daily, 30 minutes after the same meal each day

•      Swallow capsule whole; do not crush, chew, or open the modified-release capsule

•      Blood pressure monitoring is recommended at initiation in patients on antihypertensives

•      Always follow the dosage prescribed by your urologist

SIDE EFFECTS

Common:

•      Dizziness and orthostatic hypotension (first-dose effect; reduced with modified-release)

•      Abnormal ejaculation (retrograde or reduced ejaculation)

•      Rhinitis and nasal congestion

•      Headache

Less common:

•      Intraoperative floppy iris syndrome (IFIS) during cataract surgery; inform ophthalmologist before any eye procedure

•      Priapism (rare; seek immediate medical attention)

•      Palpitations

•      Rash and pruritus

NOT RECOMMENDED IN

•      History of orthostatic hypotension

•      Concurrent use with strong CYP3A4 or CYP2D6 inhibitors without monitoring

•      Severe hepatic impairment

•      Not for use in women or children

DRUG INTERACTIONS

Strong CYP3A4 inhibitors such as ketoconazole and fluconazole increase tamsulosin exposure significantly. CYP2D6 inhibitors such as paroxetine increase tamsulosin levels. PDE5 inhibitors combined with tamsulosin may cause additive blood pressure reduction; co-administer with caution. Other alpha-blockers should not be combined. Warfarin plasma levels may be slightly increased by tamsulosin.
