INDICATION

Tasmi 80mg Tab is an angiotensin II receptor blocker (ARB) indicated for the treatment of essential hypertension and for the reduction of cardiovascular mortality in patients with established atherosclerotic cardiovascular disease or type 2 diabetes with target organ damage who cannot tolerate ACE inhibitors.

MECHANISM OF ACTION

Telmisartan selectively blocks the angiotensin II type 1 (AT1) receptor, preventing angiotensin II-mediated vasoconstriction, aldosterone release, and renal sodium retention. It provides 24-hour blood pressure control with once-daily dosing and has a longer receptor binding duration than most other ARBs. Telmisartan also has partial PPAR-gamma agonist activity, which may contribute to improvements in insulin sensitivity and metabolic parameters. The 80mg dose is the maximum recommended standard dose and provides optimal AT1 blockade in most patients.

DOSAGE AND ADMINISTRATION

•      Hypertension: 80mg once daily

•      Cardiovascular risk reduction: 80mg once daily

•      Take at the same time each day, with or without food

•      Always follow the dosage and blood pressure monitoring schedule prescribed by your doctor

SIDE EFFECTS

Common:

•      Dizziness and headache

•      Fatigue

•      Hypotension, particularly on first dose or in volume-depleted patients

•      Back pain and arthralgia

Less common:

•      Hyperkalemia, particularly in renal impairment or with potassium supplements

•      Elevated creatinine (renal function worsening)

•      Angioedema (rare)

•      Urinary tract infection

NOT RECOMMENDED IN

•      Pregnancy (telmisartan causes fetal harm; contraindicated)

•      Concomitant use with Aliskiren in patients with diabetes or renal impairment

•      Severe hepatic impairment or biliary obstructive disorders (telmisartan is primarily biliary excreted)

•      Known hypersensitivity to telmisartan

DRUG INTERACTIONS

Potassium-sparing diuretics, potassium supplements, and other RAAS agents increase hyperkalemia risk. NSAIDs reduce antihypertensive efficacy and may worsen renal function. Lithium clearance may be reduced; monitor lithium levels. Aliskiren combination is contraindicated in diabetics. Ramipril combined with telmisartan increases adverse effects without additional cardiovascular benefit.
