INDICATION

Tamiflu 75mg Cap is a neuraminidase inhibitor antiviral agent indicated for the treatment of uncomplicated influenza A and B in adults and children aged 1 year and older who have been symptomatic for no more than 48 hours. It is also indicated for the prevention (prophylaxis) of influenza in adults and children aged 1 year and older following close contact with an infected individual.

MECHANISM OF ACTION

Oseltamivir is a prodrug that is extensively converted in the liver to oseltamivir carboxylate, the active inhibitor of influenza neuraminidase. Neuraminidase is an enzyme on the surface of influenza virions that cleaves sialic acid residues on host cell receptors, allowing new viral particles to be released from infected cells and spread to uninfected cells. By inhibiting neuraminidase, oseltamivir prevents viral release and limits the spread of infection, reducing symptom severity, illness duration, and the risk of serious complications such as pneumonia.

DOSAGE AND ADMINISTRATION

•      Treatment: 75mg twice daily for 5 days, starting within 48 hours of symptom onset

•      Prophylaxis: 75mg once daily for at least 10 days after exposure

•      May be taken with or without food; taking with food reduces nausea

•      Always follow the dosage prescribed by your doctor; start treatment as early as possible for maximum benefit

SIDE EFFECTS

Common:

•      Nausea and vomiting (most common; reduced when taken with food)

•      Headache

•      Abdominal pain

•      Diarrhoea

Less common:

•      Neuropsychiatric events including confusion and abnormal behavior (particularly in children and adolescents; monitor closely)

•      Stevens-Johnson syndrome (rare serious skin reaction)

•      Elevated liver enzymes

•      Anaphylaxis (rare)

NOT RECOMMENDED IN

•      Known hypersensitivity to oseltamivir or any excipient

•      Not for treatment of infections caused by other respiratory pathogens that are not influenza

•      Use with caution in severe renal impairment (dose reduction required)

•      Not a substitute for influenza vaccination

DRUG INTERACTIONS

Probenecid reduces renal tubular secretion of oseltamivir carboxylate, approximately doubling its plasma concentration; avoid concurrent use or monitor for toxicity. Live attenuated influenza vaccine (nasal spray) should not be administered within 2 weeks before or 48 hours after oseltamivir, as the antiviral effect may impair vaccine replication. No significant pharmacokinetic interactions with other common medications have been identified.
