INDICATION

Tagipmet XR 100mg/1000mg Tab is a high-dose extended-release fixed combination DPP-4 inhibitor and biguanide indicated for type 2 diabetes mellitus in adults who require sitagliptin 100mg (the full standard daily dose) combined with metformin 1000mg in a once-daily extended-release formulation.

MECHANISM OF ACTION

Sitagliptin at 100mg provides full DPP-4 inhibition, maximally increasing active incretin levels for glucose-dependent insulin secretion and glucagon suppression throughout the day. The extended-release metformin 1000mg is gradually released in the lower gastrointestinal tract, reducing peak plasma concentrations and gastrointestinal side effects while maintaining full glycaemic efficacy. Once-daily dosing simplifies the treatment regimen and may improve adherence.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet once daily with the evening meal

•      Swallow whole; do not crush, split, or chew extended-release tablets

•      Monitor renal function before initiation and regularly

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Diarrhoea and nausea (reduced with XR compared to immediate-release)

•      Headache

•      Nasopharyngitis (sitagliptin-related)

•      Vitamin B12 deficiency

DRUG INTERACTIONS

Same as other sitagliptin-metformin combinations. Iodinated contrast: stop metformin 48 hours before. Alcohol: lactic acidosis risk. Insulin and sulfonylureas: hypoglycemia risk. Cationic drugs reduce metformin renal clearance.
