INDICATION

Stork 400mg Cap is a third-generation oral cephalosporin antibiotic indicated for the treatment of mild to moderate bacterial infections caused by susceptible organisms, including uncomplicated urinary tract infections, acute exacerbations of chronic bronchitis, community-acquired pneumonia, pharyngitis and tonsillitis caused by Streptococcus pyogenes, and uncomplicated gonorrhea.

MECHANISM OF ACTION

Cefixime is a bactericidal third-generation oral cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins and blocking peptidoglycan transpeptidation. It is stable against many beta-lactamases produced by gram-negative organisms, providing broader gram-negative activity than first and second-generation cephalosporins. It is active against Haemophilus influenzae, Moraxella catarrhalis, Escherichia coli, Proteus mirabilis, and Streptococcus pyogenes. The 400mg dose provides therapeutic concentrations in urine, respiratory secretions, and most body tissues.

DOSAGE AND ADMINISTRATION

•      Adults: 400mg once daily or 200mg twice daily

•      Uncomplicated gonorrhea: 400mg as a single dose

•      Take with or without food; food may reduce nausea

•      Always complete the full prescribed course as directed by your doctor

SIDE EFFECTS

Common:

•      Diarrhoea (most common; more frequent than with other oral cephalosporins)

•      Nausea and abdominal discomfort

•      Headache

•      Skin rash or urticaria

Less common:

•      Hypersensitivity reactions including anaphylaxis in penicillin-allergic patients

•      Clostridium difficile-associated diarrhoea with prolonged use

•      Elevated liver enzymes

•      Thrombocytopenia with prolonged use

NOT RECOMMENDED IN

•      Known hypersensitivity to cephalosporins or penicillins (cross-reactivity possible)

•      Severe renal impairment (dose reduction required; CrCl below 20 ml/min: 200mg once daily)

•      Not effective against Pseudomonas aeruginosa or MRSA

•      Caution in patients with history of gastrointestinal disease

DRUG INTERACTIONS

Probenecid reduces renal tubular secretion of cefixime, increasing plasma levels and prolonging half-life. Warfarin anticoagulant effect may be enhanced during antibiotic therapy; monitor INR closely. Antacids and H2-blockers do not significantly affect cefixime absorption. Carbamazepine levels may be increased by cefixime; monitor carbamazepine plasma concentrations in patients on antiepileptic therapy.
