INDICATION

Stir-Up 10mg Tab is an NMDA receptor antagonist indicated for the treatment of moderate to severe Alzheimer's dementia. It is used to improve or stabilise cognitive function, daily activities, and behavior in patients with established Alzheimer's disease who are not adequately managed on cholinesterase inhibitor therapy alone or who cannot tolerate it.

MECHANISM OF ACTION

Memantine is a low-affinity, voltage-dependent, uncompetitive antagonist of N-methyl-D-aspartate (NMDA) receptors. In Alzheimer's disease, excessive glutamate activity leads to chronic low-level activation of NMDA receptors, causing sustained calcium influx and neuronal excitotoxicity that damages neurons involved in memory and cognition. Memantine blocks open NMDA receptor channels in a voltage-dependent manner, preventing this pathological tonic activation while still allowing normal synaptic NMDA receptor function required for learning and memory. This selective blockade reduces excitotoxic neuronal damage without significantly impairing physiological glutamatergic transmission.

DOSAGE AND ADMINISTRATION

•      Initial dose: 5mg once daily in the first week

•      Week 2: increase to 10mg per day (5mg twice daily); week 3: 15mg per day; week 4 onwards: 20mg per day (10mg twice daily)

•      Target maintenance dose: 10mg twice daily (20mg per day)

•      Always follow the titration schedule prescribed by your neurologist; dose reduction required in moderate renal impairment

SIDE EFFECTS

Common:

•      Dizziness and headache

•      Constipation

•      Somnolence and fatigue

•      Hypertension

Less common:

•      Confusion and hallucinations (particularly in patients with severe dementia)

•      Vomiting

•      Gait disturbance

•      Elevated liver enzymes

NOT RECOMMENDED IN

•      Known hypersensitivity to memantine or any excipient

•      Severe renal impairment (eGFR below 5 ml/min; limited safety data)

•      Conditions that raise urinary pH such as renal tubular acidosis or severe urinary tract infections (reduce memantine elimination)

•      Caution in patients with epilepsy or recent myocardial infarction

DRUG INTERACTIONS

NMDA antagonists such as amantadine, ketamine, and dextromethorphan should not be combined with memantine due to additive NMDA receptor blockade and increased psychotomimetic risk. Drugs that alkalinize urine, including carbonic anhydrase inhibitors and sodium bicarbonate, reduce renal clearance of memantine and increase plasma levels; monitor for toxicity. L-dopa, dopaminergic agonists, and anticholinergic drugs may have enhanced effects when combined with memantine. Warfarin INR may be increased; monitor in patients on anticoagulants.
