INDICATION

Sporizol 100mg Cap is a broad-spectrum triazole antifungal indicated for the treatment of systemic and superficial fungal infections including aspergillosis, candidiasis, cryptococcosis, histoplasmosis, onychomycosis (nail fungus), tinea versicolor, tinea corporis, tinea pedis, and pityriasis versicolor.

MECHANISM OF ACTION

Itraconazole inhibits the fungal cytochrome P450 enzyme lanosterol 14-alpha-demethylase, which is required for the conversion of lanosterol to ergosterol, the primary sterol of the fungal cell membrane. Depletion of ergosterol accumulates toxic sterols in the fungal membrane, impairing membrane integrity and permeability, ultimately causing fungal cell death. Itraconazole has a broader spectrum than fluconazole, including activity against Aspergillus species and dermatophytes, and its highly lipophilic nature ensures excellent penetration into skin, nails, and lung tissue.

DOSAGE AND ADMINISTRATION

•      Onychomycosis (pulse therapy): 200mg twice daily for 1 week per month for 2 to 3 months

•      Systemic fungal infections: 100 to 200mg once or twice daily for 3 months

•      Take capsules with a full meal to maximize absorption

•      Always follow the dosage and duration prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea, abdominal pain, dyspepsia

•      Headache and dizziness

•      Elevated liver enzymes

•      Skin rash

Less common:

•      Hepatotoxicity with prolonged therapy (monitor liver function)

•      Cardiac failure with negative inotropic effect at higher doses

•      Peripheral neuropathy with prolonged use

•      Hypokalaemia

NOT RECOMMENDED IN

•      Known hypersensitivity to itraconazole or other triazole antifungals

•      Heart failure (negative inotropic effect)

•      Pregnancy (teratogenic potential; use reliable contraception)

•      Concurrent use with drugs that are contraindicated with strong CYP3A4 inhibitors

DRUG INTERACTIONS

Itraconazole is a potent CYP3A4 inhibitor and significantly increases plasma levels of statins (rhabdomyolysis risk; avoid simvastatin and lovastatin), cyclosporine, tacrolimus, digoxin, and many other drugs. It is contraindicated with cisapride, pimozide, quinidine, ergot alkaloids, and terfenadine. H2-blockers and PPIs reduce itraconazole absorption. Rifampicin and carbamazepine markedly reduce itraconazole levels.
