INDICATION

Smash-2 75mg Tab is a combination NSAID and synthetic prostaglandin indicated for the treatment of pain and inflammation in osteoarthritis, rheumatoid arthritis, and other musculoskeletal conditions in patients who require NSAID therapy and are at risk of developing NSAID-induced gastric and duodenal ulcers. The misoprostol component provides gastroprotection against diclofenac-related gastrointestinal injury.

MECHANISM OF ACTION

Diclofenac inhibits both cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, reducing prostaglandin synthesis and thereby decreasing inflammation, pain, and fever. However, COX-1 inhibition reduces gastric mucosal prostaglandins that normally maintain mucosal integrity, increasing ulcer risk. Misoprostol is a synthetic prostaglandin E1 analogue that directly activates EP receptors in the gastric mucosa, stimulating mucus and bicarbonate secretion, maintaining mucosal blood flow, and inhibiting acid secretion. This restores the cytoprotective prostaglandin activity suppressed by diclofenac, significantly reducing the incidence of NSAID-induced gastric and duodenal ulcers.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet two to three times daily with food

•      Take with meals to reduce gastrointestinal side effects from both components

•      Use the lowest effective dose for the shortest duration necessary

•      Always follow the dosage and duration prescribed by your doctor; avoid in patients not requiring gastroprotection

SIDE EFFECTS

Common:

•      Diarrhoea and abdominal cramps (misoprostol-related; most common side effect)

Less common:

•      Uterine cramping and bleeding (misoprostol stimulates uterine contractions; contraindicated in pregnancy)

•      Renal impairment with prolonged NSAID use

NOT RECOMMENDED IN

•      Pregnancy: misoprostol causes uterine contractions and can induce miscarriage or preterm labor; absolutely contraindicated

•      Women of childbearing potential not using reliable contraception

•      Active peptic ulcer or gastrointestinal bleeding

•      Severe renal or hepatic impairment; known hypersensitivity to diclofenac, misoprostol, or prostaglandins

DRUG INTERACTIONS

Concurrent NSAIDs or aspirin at analgesic doses increase gastrointestinal bleeding risk. Anticoagulants such as warfarin have enhanced effect with diclofenac; monitor INR. ACE inhibitors, ARBs, and diuretics may lose antihypertensive efficacy and renal function may worsen with concurrent NSAID use. Antacids containing magnesium reduce the rate but not extent of misoprostol absorption; avoid magnesium-containing antacids in patients with renal impairment. Diclofenac inhibits CYP2C9 and may increase plasma levels of certain anticoagulants and sulfonylureas metabolized by this enzyme.
