INDICATION
Sivab 5mg Tab is a selective funny current (If) inhibitor indicated for the symptomatic treatment of chronic stable angina pectoris in adults with normal sinus rhythm who have a heart rate above 70 beats per minute and cannot tolerate or have a contraindication to beta-blockers. It is also indicated for heart failure with reduced ejection fraction (HFrEF) in patients with a heart rate above 75 beats per minute.
MECHANISM OF ACTION
Ivabradine selectively and dose-dependently inhibits the cardiac pacemaker current (If or funny current) in the sinoatrial node, which is responsible for the spontaneous rhythmic depolarization of pacemaker cells. By blocking If channels, ivabradine reduces heart rate in a pure, dose-dependent manner without affecting cardiac contractility, blood pressure, or conduction. The reduction in heart rate decreases myocardial oxygen demand and increases diastolic filling time and coronary blood flow, reducing angina frequency. Its selectivity for the SA node means it has no direct effect on ventricular repolarization.
DOSAGE AND ADMINISTRATION
• Stable angina and heart failure: 5mg twice daily with meals initially
• If heart rate remains above 60 beats per minute after 3 to 4 weeks, dose may be increased to 7.5mg twice daily
• If resting heart rate falls below 50 beats per minute, reduce dose to 2.5mg twice daily
• Always follow the dose titration based on resting heart rate as prescribed by your cardiologist
SIDE EFFECTS
Common:
• Phosphenes (transient visual brightness or luminous phenomena; benign, occur with eye exposure to light changes)
• Bradycardia (slow heart rate) if dose is too high
• Headache and dizziness
• Atrial fibrillation (monitor; increased risk with long-term use)
Less common:
• AV block (heart conduction disturbance)
• Hypertension (rebound if stopped abruptly)
• Nausea
• Elevated uric acid levels
NOT RECOMMENDED IN
• Resting heart rate below 70 beats per minute before initiating (angina) or below 75 bpm (heart failure)
• Acute myocardial infarction or unstable angina
• Sick sinus syndrome or sinoatrial block without a functioning pacemaker
• Concurrent strong CYP3A4 inhibitors such as ketoconazole, verapamil, diltiazem, and clarithromycin
DRUG INTERACTIONS
Strong CYP3A4 inhibitors including ketoconazole, ritonavir, and clarithromycin markedly increase ivabradine plasma levels, causing excessive bradycardia; contraindicated. Verapamil and diltiazem are contraindicated due to heart rate reduction and CYP3A4 inhibition. CYP3A4 inducers such as rifampicin and carbamazepine reduce ivabradine efficacy. QT-prolonging drugs may worsen cardiac rhythm; use with caution.
