INDICATION

Sitarmin 50/1000mg Tab is a fixed-dose combination DPP-4 inhibitor and biguanide antidiabetic indicated as adjunctive therapy to diet and exercise in adults with type 2 diabetes mellitus inadequately controlled on metformin 1000mg alone or on sitagliptin and metformin given separately.

MECHANISM OF ACTION

Sitagliptin inhibits DPP-4 to enhance glucose-dependent incretin-mediated insulin secretion and suppress glucagon. Metformin 1000mg reduces hepatic glucose output, improves insulin sensitivity in peripheral tissues, and reduces intestinal glucose absorption. Together, they address multiple pathophysiological defects in type 2 diabetes with complementary and additive glycaemic benefit, reducing both fasting and postprandial glucose without causing weight gain or hypoglycemia as monotherapy.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet twice daily with meals

•      Renal function assessment is mandatory before initiation and periodically

•      May be taken with food to improve metformin tolerability

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea (metformin-related)

•      Nasopharyngitis

•      Headache

•      Abdominal discomfort

Less common:

•      Lactic acidosis (rare; risk in renal impairment)

•      Pancreatitis (rare)

•      Vitamin B12 deficiency

•      Hypoglycemia with concurrent insulin or sulfonylurea

•      eGFR below 45 ml/min/1.73m2

DRUG INTERACTIONS

Same interaction profile as other sitagliptin-metformin combinations. Iodinated contrast media: stop metformin 48 hours before. Alcohol increases lactic acidosis risk. Cationic drugs reduce metformin clearance. Insulin and sulfonylureas: hypoglycemia risk.
