INDICATION

Sita-Met XR 50/1000mg Tab is a fixed-dose extended-release combination of sitagliptin and metformin indicated for type 2 diabetes mellitus in adults. The extended-release formulation improves gastrointestinal tolerability of metformin and allows once-daily dosing, improving treatment adherence.

MECHANISM OF ACTION

Sitagliptin inhibits DPP-4, increasing active incretin levels and enhancing glucose-dependent insulin secretion. Metformin in extended-release form is gradually released in the gastrointestinal tract, reaching the colon where the main absorption site is located. This controlled release reduces peak plasma concentrations and gastrointestinal side effects, particularly nausea and diarrhoea, while maintaining equivalent glycaemic efficacy to immediate-release metformin.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet once daily with the evening meal

•      Swallow whole; do not crush, split, or chew the extended-release tablet

•      Monitor renal function before initiation and periodically

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Diarrhoea and nausea (reduced compared to immediate-release)

•      Headache

•      Nasopharyngitis (sitagliptin-related)

•      Abdominal discomfort

Less common:

•      Lactic acidosis (rare; mainly in severe renal impairment)

•      Pancreatitis (rare)

•      Vitamin B12 deficiency with long-term metformin use

DRUG INTERACTIONS

Iodinated contrast media: stop metformin before procedure. Alcohol increases lactic acidosis risk. Cimetidine reduces metformin renal clearance. Insulin and sulfonylureas: reduce dose to avoid hypoglycemia. ACE inhibitors may enhance DPP-4 inhibitor effect.
