INDICATION

Sitaglu Met 50mg+1000mg Tab is a fixed-dose combination DPP-4 inhibitor and biguanide antidiabetic indicated as an adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes mellitus who are inadequately controlled on metformin alone or on sitagliptin and metformin as separate tablets.

MECHANISM OF ACTION

Sitagliptin enhances incretin-mediated, glucose-dependent insulin secretion and suppresses glucagon, working particularly at postprandial glucose peaks. Metformin reduces hepatic glucose production via AMPK activation, improves peripheral insulin sensitivity, and reduces intestinal glucose absorption without causing hypoglycemia or weight gain. The combination addresses multiple pathophysiological mechanisms of type 2 diabetes with complementary and additive glycaemic benefit.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet twice daily with meals (morning and evening)

•      The 1000mg metformin dose is the higher-dose formulation for patients needing greater metformin exposure

•      Monitor renal function before starting and regularly during treatment

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea (metformin-related; take with food)

DRUG INTERACTIONS

Iodinated contrast media combined with metformin can cause acute renal failure and lactic acidosis. Alcohol increases lactic acidosis risk. Cationic drugs such as cimetidine reduce renal tubular secretion of metformin, increasing plasma levels. ACE inhibitors may enhance glucose-lowering with sitagliptin. Insulin and sulfonylureas add hypoglycemia risk.
