INDICATION

Sitaglu 50mg Tab is a dipeptidyl peptidase-4 (DPP-4) inhibitor indicated for the treatment of type 2 diabetes mellitus in adults, as monotherapy or in combination with other antidiabetic agents including metformin, sulfonylureas, and insulin when diet and exercise are insufficient for adequate glycaemic control.

MECHANISM OF ACTION

Sitagliptin inhibits DPP-4, the enzyme that rapidly degrades the incretin hormones GLP-1 and GIP after their release from the intestine in response to food. By blocking DPP-4, sitagliptin increases active incretin levels, which stimulate glucose-dependent insulin secretion from pancreatic beta cells and suppress inappropriately elevated glucagon secretion from alpha cells, reducing hepatic glucose output. This glucose-dependent mechanism of action does not cause hypoglycemia when used as monotherapy. The 50mg dose is used in patients with moderate renal impairment or as a component of fixed-dose combination tablets.

DOSAGE AND ADMINISTRATION

•      Type 2 diabetes (moderate renal impairment CrCl 30 to 50 ml/min): 50mg once daily

•      As a component of combination regimens: dose and frequency as prescribed

•      May be taken with or without food

•      Always follow the dosage and renal function monitoring schedule prescribed by your doctor

SIDE EFFECTS

Common:

•      Nasopharyngitis (common cold symptoms)

•      Headache

•      Upper respiratory tract infection

•      Mild nausea

Less common:

•      Pancreatitis (rare; discontinue if persistent severe abdominal pain develops)

•      Hypersensitivity reactions including rash, urticaria, and angioedema

•      Joint pain (arthralgia)

•      Bullous pemphigoid (rare skin blistering condition)

NOT RECOMMENDED IN

•      Severe renal impairment (eGFR below 30 ml/min) or end-stage renal disease without further dose reduction

•      Type 1 diabetes or diabetic ketoacidosis

•      Known hypersensitivity to sitagliptin

•      History of pancreatitis; use with caution

DRUG INTERACTIONS

Sitagliptin is primarily excreted unchanged by the kidneys and has minimal CYP450 interactions. Digoxin plasma levels may be slightly increased with sitagliptin; monitor in patients on digoxin. Sulfonylureas and insulin used concurrently may cause hypoglycemia; reduce sulfonylurea or insulin dose as required. Cyclosporine modestly increases sitagliptin AUC through P-glycoprotein and renal transporter inhibition.
