INDICATION

Silorap 4mg Cap is a selective alpha-1A adrenergic receptor antagonist indicated for the symptomatic treatment of benign prostatic hyperplasia (BPH) in adult men. It improves urinary flow and reduces lower urinary tract symptoms including hesitancy, weak stream, nocturia, and urgency.

MECHANISM OF ACTION

Silodosin selectively blocks alpha-1A receptors in the prostate, bladder neck, and urethra, relaxing smooth muscle and reducing urethral resistance without significantly affecting vascular blood pressure. Its high selectivity for the prostatic alpha-1A subtype over the vascular alpha-1B subtype minimizes cardiovascular side effects. The 4mg dose provides effective symptom relief while reducing the incidence of ejaculatory dysfunction compared to the 8mg dose.

DOSAGE AND ADMINISTRATION

•      Adults: 4mg once daily with the main meal

•      Use the 4mg dose in patients with moderate renal impairment or those sensitive to alpha-blockers

•      Take at the same time each day to maintain consistent drug levels

•      Always follow the dosage prescribed by your urologist

SIDE EFFECTS

Common:

•      Retrograde ejaculation

•      Dizziness, particularly on first dose

•      Nasal congestion

•      Diarrhoea

Less common:

•      Intraoperative floppy iris syndrome (IFIS) during cataract surgery; inform eye surgeon

•      Orthostatic hypotension

•      Headache

•      Palpitations

NOT RECOMMENDED IN

•      Severe renal impairment (CrCl below 30 ml/min)

•      Severe hepatic impairment

•      Strong CYP3A4 inhibitors

•      Not for use in women or children

DRUG INTERACTIONS

Strong CYP3A4 inhibitors markedly increase silodosin plasma levels; avoid. PDE5 inhibitors may cause additive blood pressure reduction. P-glycoprotein inhibitors also increase silodosin exposure. Antihypertensives may have additive hypotensive effect.
