INDICATION

Sildoso 4mg Cap is a highly selective alpha-1A adrenergic receptor blocker indicated for the treatment of signs and symptoms of benign prostatic hyperplasia (BPH) in adult men. It relieves lower urinary tract symptoms including weak urinary stream, urinary hesitancy, incomplete bladder emptying, urgency, and nocturia.

MECHANISM OF ACTION

Silodosin selectively blocks alpha-1A adrenergic receptors located predominantly in the prostate gland, bladder neck, and urethra, with minimal effect on vascular alpha-1B receptors. This high receptor selectivity relaxes smooth muscle in the prostate and bladder outlet without causing significant blood pressure reduction, reducing urethral resistance and improving urinary flow rate. Its selectivity for the alpha-1A subtype results in fewer cardiovascular side effects compared to less selective alpha-blockers such as tamsulosin. The 4mg dose is the lower starting dose used in patients with moderate renal impairment or those sensitive to alpha-blockers.

DOSAGE AND ADMINISTRATION

•      Adults: 4mg once daily with a meal

•      Patients with moderate renal impairment (CrCl 30 to 50 ml/min): 4mg once daily (do not increase to 8mg)

•      Take at the same time each day with food to reduce dizziness

•      Always follow the dosage prescribed by your urologist

SIDE EFFECTS

Common:

•      Retrograde ejaculation (most common; harmless but affects fertility temporarily)

•      Dizziness and orthostatic hypotension, particularly at initiation

•      Nasal congestion

•      Diarrhoea

Less common:

•      Intraoperative floppy iris syndrome (IFIS) during cataract surgery; inform ophthalmologist before any eye procedure

•      Syncope on standing in sensitive patients

•      Headache

•      Elevated heart rate

NOT RECOMMENDED IN

•      Severe renal impairment (CrCl below 30 ml/min)

•      Severe hepatic impairment

•      Concurrent use with strong CYP3A4 inhibitors or P-glycoprotein inhibitors

•      Known hypersensitivity to silodosin; not for use in women or children

DRUG INTERACTIONS

Strong CYP3A4 inhibitors such as ketoconazole, ritonavir, and clarithromycin significantly increase silodosin plasma levels; avoid combination. PDE5 inhibitors used concurrently with silodosin may cause additional blood pressure reduction; co-administer with caution. Strong P-glycoprotein inhibitors also increase silodosin exposure. Antihypertensive drugs may have additive hypotensive effects.
