INDICATION
Silage 50mg Tab is a substituted benzamide selective dopamine D2 receptor antagonist indicated for the symptomatic treatment of functional dyspepsia, gastroparesis, nausea, and vomiting associated with upper gastrointestinal motility disorders. It enhances gastric emptying and reduces symptoms such as bloating, early satiety, nausea, and epigastric pain.
MECHANISM OF ACTION
Levosulpiride is the levorotatory enantiomer of sulpiride and acts as a selective antagonist of dopamine D2 receptors in the gastrointestinal tract and the chemoreceptor trigger zone. Peripheral D2 receptor blockade in the myenteric plexus enhances acetylcholine release, stimulating gastric peristalsis and accelerating gastric emptying. Central D2 blockade in the chemoreceptor trigger zone provides antiemetic effects. Levosulpiride has greater peripheral selectivity and higher potency than racemic sulpiride, providing effective prokinetic and antiemetic activity at lower doses.
DOSAGE AND ADMINISTRATION
• Adults: 50mg two to three times daily, 30 minutes before meals
• Do not exceed 150mg per day
• Duration of treatment typically 4 to 8 weeks for functional dyspepsia
• Always follow the dosage and duration prescribed by your gastroenterologist or physician
SIDE EFFECTS
Common:
• Somnolence and drowsiness
• Hyperprolactinemia: galactorrhea, amenorrhea, and breast tenderness
• Restlessness (akathisia)
• Nausea initially during the first few days of use
Less common:
• Extrapyramidal symptoms: tardive dyskinesia with prolonged use
• QT prolongation at higher doses
• Sexual dysfunction related to elevated prolactin
• Weight gain with prolonged use
NOT RECOMMENDED IN
• Pheochromocytoma (dopamine-dependent tumor)
• Parkinson's disease and Lewy body dementia
• Prolactin-dependent tumors such as prolactinoma or prolactin-dependent breast cancer
• Known hypersensitivity to sulpiride or Levosulpiride
DRUG INTERACTIONS
Concurrent use with other dopamine antagonists including antipsychotics and antiemetics increases the risk of extrapyramidal side effects and hyperprolactinemia. QT-prolonging drugs including antiarrhythmics, macrolide antibiotics, and fluoroquinolones increase the risk of cardiac arrhythmia; avoid concurrent use. CNS depressants and alcohol have additive sedative effects. Levodopa and dopamine agonists used in Parkinson's disease are pharmacologically opposed by Levosulpiride; avoid concurrent use.
