INDICATION

Sensival 25mg Tab is a tricyclic antidepressant indicated for the treatment of major depressive disorder, neuropathic pain syndromes including diabetic neuropathy and postherpetic neuralgia, and migraine prophylaxis. It is also used in nocturnal enuresis in children and for the management of chronic pain conditions.

MECHANISM OF ACTION

Nortriptyline is the active metabolite of amitriptyline and a secondary amine tricyclic antidepressant. It inhibits the reuptake of norepinephrine and, to a lesser degree, serotonin into presynaptic neurons, enhancing noradrenergic and serotonergic neurotransmission. Compared to other tricyclics, nortriptyline has a more favorable tolerability profile due to relatively lower affinity for muscarinic, histaminergic, and alpha-1 adrenergic receptors. It also inhibits sodium channels in peripheral nerves, contributing to its analgesic properties in neuropathic pain. Nortriptyline has a well-established therapeutic plasma concentration range (50 to 150 ng/ml) that guides dosing.

DOSAGE AND ADMINISTRATION

•      Depression: 25mg once daily at night initially; increase to 75 to 150mg per day in divided doses or as a single nighttime dose

•      Neuropathic pain: 25mg at bedtime; titrate to 75mg per day

•      Nocturnal enuresis in children: 10 to 25mg at bedtime under pediatric supervision

•      Always follow the dosage and titration schedule prescribed by your doctor; never stop abruptly

SIDE EFFECTS

Common:

•      Dry mouth, constipation, and urinary hesitancy (anticholinergic effects)

•      Sedation and drowsiness (useful for sleep in depressed patients)

•      Orthostatic hypotension (dizziness on standing)

•      Weight gain

Less common:

•      QT prolongation and cardiac arrhythmias (monitor ECG in cardiac patients)

•      Confusion and delirium in elderly patients

•      Tachycardia

•      Tremor and fine motor incoordination

NOT RECOMMENDED IN

•      Recent myocardial infarction or serious cardiac arrhythmias

•      Concurrent use with MAO inhibitors (hypertensive crisis and serotonin syndrome)

•      Narrow-angle glaucoma

•      Known hypersensitivity to nortriptyline or other tricyclics

DRUG INTERACTIONS

MAO inhibitors are contraindicated; a 14-day washout is required. SSRIs, particularly fluoxetine and paroxetine (CYP2D6 inhibitors), significantly increase nortriptyline plasma levels; monitor for toxicity. Other QT-prolonging drugs increase cardiac arrhythmia risk. CNS depressants including alcohol and benzodiazepines add to sedation. Anticholinergic drugs produce additive anticholinergic side effects.
