INDICATION

Scrodop DR 30mg Cap is a serotonin and norepinephrine reuptake inhibitor (SNRI) indicated for the treatment of major depressive disorder, generalized anxiety disorder, diabetic peripheral neuropathic pain, fibromyalgia, and chronic musculoskeletal pain. It is also used in stress urinary incontinence.

MECHANISM OF ACTION

Duloxetine inhibits both the serotonin transporter (SERT) and the norepinephrine transporter (NET) with high potency and balanced selectivity, increasing synaptic concentrations of both neurotransmitters. The dual mechanism contributes to its antidepressant and anxiolytic effects through enhanced serotonergic and noradrenergic neurotransmission in mood-regulating circuits. Its analgesic effects in neuropathic pain arise from descending pain inhibition via noradrenergic pathways in the spinal cord. The delayed-release enteric coating protects the drug from gastric acid degradation and reduces nausea.

DOSAGE AND ADMINISTRATION

•      Depression and anxiety: 30mg once daily initially; may increase to 60mg daily after 1 to 2 weeks

•      Neuropathic pain and fibromyalgia: 30 to 60mg once daily

•      Swallow capsules whole; do not crush, open, or chew

•      Always follow the dosage and titration schedule prescribed by your doctor; never stop abruptly

SIDE EFFECTS

Common:

•      Nausea (most common, usually transient; take with food)

•      Headache and dizziness

•      Dry mouth

•      Fatigue and somnolence

Less common:

•      Elevated blood pressure (monitor regularly)

•      Sexual dysfunction

•      Sweating and tremor

•      Hyponatremia, particularly in elderly patients

NOT RECOMMENDED IN

•      Concurrent use with MAO inhibitors (serotonin syndrome; 14-day washout required)

DRUG INTERACTIONS

MAO inhibitors are contraindicated; 14-day washout required in both directions. Duloxetine is a moderate CYP2D6 inhibitor and increases plasma levels of tricyclic antidepressants, antipsychotics, and some beta-blockers metabolized by CYP2D6. Strong CYP1A2 inhibitors such as fluvoxamine and ciprofloxacin markedly increase duloxetine levels; avoid concurrent use. Concurrent serotonergic drugs including tramadol, triptans, and St John's Wort increase serotonin syndrome risk. NSAIDs and aspirin increase bleeding risk.
