INDICATION

Rovista 5mg Tab is a statin (HMG-CoA reductase inhibitor) indicated for the treatment of primary hypercholesterolemia, mixed dyslipidemia, and homozygous familial hypercholesterolemia. It is used as an adjunct to diet to reduce elevated total cholesterol, LDL-cholesterol, and triglycerides, and to raise HDL-cholesterol.

MECHANISM OF ACTION

Rosuvastatin competitively and selectively inhibits HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis in hepatocytes. By reducing intracellular cholesterol synthesis, it triggers upregulation of LDL receptors on the hepatocyte surface, increasing clearance of LDL-cholesterol from plasma. Rosuvastatin is the most potent statin on a milligram basis, with a 5mg dose producing LDL reductions of 35 to 45%. Unlike lipophilic statins, rosuvastatin's hydrophilic nature reduces non-specific tissue uptake, contributing to its favorable muscle safety profile.

DOSAGE AND ADMINISTRATION

•      Adults: 5mg once daily in the evening or at any time of day

•      Dose may be increased to 10mg, 20mg, or 40mg based on lipid response and tolerability

•      Starting dose for Asian patients, elderly patients, and those with renal impairment

•      Always follow the dosage and monitoring plan prescribed by your doctor

SIDE EFFECTS

Common:

•      Headache

•      Myalgia (muscle ache)

•      Nausea and abdominal discomfort

•      Elevated liver transaminases (ALT, AST)

Less common:

•      Myopathy or rhabdomyolysis (rare; increased risk with interacting drugs or high doses)

•      Proteinuria and hematuria (dose-dependent; monitor urine at higher doses)

•      New-onset type 2 diabetes with prolonged use

•      Arthralgia

NOT RECOMMENDED IN

•      Active liver disease or persistent elevated transaminases

•      Pregnancy and breastfeeding (teratogenic)

•      Known hypersensitivity to rosuvastatin

•      Concurrent use with cyclosporine

DRUG INTERACTIONS

Cyclosporine dramatically increases rosuvastatin AUC; combination is generally contraindicated. Fibrates, particularly gemfibrozil, increase myopathy risk; avoid concurrent use. Antacids reduce rosuvastatin absorption; take rosuvastatin 2 hours after antacids. Warfarin: monitor INR. Fusidic acid combined with statins increases myopathy risk; temporarily discontinue statin during fusidic acid course.
