INDICATION

Rolip Eze 10mg+10mg Tab is a fixed-dose combination lipid-lowering agent indicated for the treatment of primary hypercholesterolemia and mixed dyslipidemia in adults. It is prescribed when diet and lifestyle modifications alone are insufficient to achieve target LDL-cholesterol levels, and when combination therapy provides greater efficacy than either agent alone.

MECHANISM OF ACTION

Rosuvastatin is an HMG-CoA reductase inhibitor that blocks the rate-limiting step of cholesterol synthesis in the liver, reducing LDL-cholesterol production and upregulating LDL receptors on hepatocytes to increase LDL clearance from plasma. Ezetimibe selectively inhibits the Niemann-Pick C1-Like 1 (NPC1L1) transporter at the intestinal brush border, blocking the absorption of dietary and biliary cholesterol from the small intestine. The combination of hepatic synthesis inhibition (statin) and intestinal absorption inhibition (ezetimibe) provides complementary and additive LDL-lowering effects of approximately 60 to 70% reduction.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet once daily, at any time of day with or without food

•      Swallow whole with water

•      Regular lipid profile monitoring is recommended every 6 to 12 weeks initially

•      Always follow the dosage and monitoring schedule prescribed by your doctor

SIDE EFFECTS

Common:

•      Headache

•      Myalgia (muscle aches) from rosuvastatin component

•      Nausea and abdominal discomfort

•      Elevated liver enzymes (transaminases)

Less common:

•      Myopathy or rhabdomyolysis at high rosuvastatin doses or with interacting drugs

•      Proteinuria with rosuvastatin at higher doses

•      Arthralgia

•      Diabetes risk with long-term statin use

NOT RECOMMENDED IN

•      Active liver disease or unexplained persistent elevated transaminases

•      Pregnancy and breastfeeding (statins are teratogenic)

•      Known hypersensitivity to rosuvastatin or ezetimibe

•      Severe renal impairment (eGFR below 30 ml/min) without dose adjustment

DRUG INTERACTIONS

Cyclosporine significantly increases rosuvastatin plasma levels; avoid combination or limit rosuvastatin dose. Fibrates, particularly gemfibrozil, increase myopathy risk with rosuvastatin; avoid concurrent use. Antacids containing magnesium and aluminium reduce ezetimibe absorption; take 2 hours apart. Bile acid sequestrants reduce ezetimibe absorption; take ezetimibe at least 2 hours before or 4 hours after. Warfarin anticoagulant effect may be enhanced; monitor INR.
