INDICATION
Riveme 1.5mg Cap is a cholinesterase inhibitor indicated for the symptomatic treatment of mild to moderately severe Alzheimer's dementia and dementia associated with Parkinson's disease. It helps maintain cognitive function, reduce behavioral symptoms, and improve activities of daily living in affected patients.
MECHANISM OF ACTION
Rivastigmine is a dual inhibitor of acetylcholinesterase (AChE) and butyrylcholinesterase (BuChE) in the central nervous system. By inhibiting both enzymes, it prevents the breakdown of acetylcholine at cholinergic synapses, increasing the availability of acetylcholine in the brain regions associated with memory and cognition. Unlike some other cholinesterase inhibitors, rivastigmine is pseudo-irreversibly inhibiting these enzymes at the active site through carbamylation, providing a sustained inhibitory effect. It is especially effective in the cortex and hippocampus, where cholinergic deficits are most pronounced in Alzheimer's disease.
DOSAGE AND ADMINISTRATION
• Initial dose: 1.5mg twice daily with morning and evening meals
• After 2 weeks, increase to 3mg twice daily; further titration to 4.5mg then 6mg twice daily at 2-week intervals as tolerated
• Maximum dose: 6mg twice daily (12mg/day)
• Always follow the titration schedule prescribed by your neurologist; do not discontinue abruptly without medical advice
SIDE EFFECTS
Common:
• Nausea and vomiting (most common, especially during titration; take with food to reduce)
• Diarrhoea and loss of appetite
• Dizziness and headache
• Fatigue and weakness
Less common:
• Significant weight loss with prolonged use
• Bradycardia (slow heart rate) due to cholinergic stimulation
• Urinary incontinence or increased frequency
• Agitation and confusion (paradoxical, particularly in fast titration)
NOT RECOMMENDED IN
• Known hypersensitivity to rivastigmine, carbamate derivatives, or any excipient
• Severe hepatic or renal impairment
• Sick sinus syndrome or severe heart conduction abnormalities
• Active peptic ulcer disease or urinary obstruction (cholinergic exacerbation)
DRUG INTERACTIONS
Rivastigmine may enhance the effects of muscle relaxants during anaesthesia due to inhibition of plasma cholinesterase (succinylcholine toxicity risk); inform surgeon before any procedure. Concurrent use with other cholinomimetics or anticholinergic drugs produces opposing effects; avoid concurrent anticholinergics. Beta-blockers combined with rivastigmine increase bradycardia risk. NSAIDs may exacerbate gastrointestinal side effects.
