INDICATION

Ribazole 400mg Cap is an antiviral agent indicated for the treatment of chronic hepatitis C virus (HCV) infection in adults, used in combination with pegylated interferon alfa or direct-acting antiviral agents (DAAs). It is never used as monotherapy for hepatitis C as it has no significant antiviral effect when used alone.

MECHANISM OF ACTION

Ribavirin is a synthetic nucleoside analogue that is phosphorylated intracellularly to its active triphosphate form. It inhibits the RNA-dependent RNA polymerase of HCV, impairing viral replication. It also has immunomodulatory properties, shifting the immune response from a Th2 to a Th1 profile, which enhances viral clearance. Additionally, ribavirin depletes intracellular GTP pools by inhibiting inosine monophosphate dehydrogenase (IMPDH), further reducing viral RNA synthesis. Its combination with interferon or DAAs significantly improves sustained virological response (SVR) rates in chronic HCV infection.

DOSAGE AND ADMINISTRATION

•      Adults under 75kg: 400mg in the morning and 600mg in the evening (1000mg/day)

•      Adults 75kg and above: 600mg twice daily (1200mg/day)

•      Take with food to improve absorption and reduce gastrointestinal side effects

•      Always follow the combination regimen and duration prescribed by your hepatologist; never take ribavirin alone for hepatitis C

SIDE EFFECTS

Common:

•      Hemolytic anemia (most significant; monitor hemoglobin closely throughout therapy)

•      Fatigue and weakness

•      Nausea, anorexia, and weight loss

•      Headache and insomnia

Less common:

•      Severe teratogenicity and embryo lethality (extreme caution; two forms of contraception mandatory for both sexes during and for 6 months after therapy)

•      Worsening of cardiac disease due to hemolytic anemia

•      Rash, pruritus, and dry skin

•      Depression and mood disturbances (potentiated by interferon)

NOT RECOMMENDED IN

•      Pregnancy and women of childbearing potential or their male partners not using two reliable contraceptive methods simultaneously

•      Haemoglobinopathies such as thalassemia and sickle cell anemia

•      Severe renal impairment (eGFR below 50 mL/min; ribavirin accumulates causing severe hemolysis)

•      Known hypersensitivity to ribavirin or any excipient

DRUG INTERACTIONS

Ribavirin inhibits phosphorylation of stavudine and zidovudine, reducing their antiviral efficacy; avoid concurrent use in HIV-HCV coinfected patients on these agents. Didanosine combined with ribavirin increases the risk of severe mitochondrial toxicity including lactic acidosis, pancreatitis, and hepatic decompensation; this combination is contraindicated. Azathioprine combined with ribavirin may cause severe pancytopenia through additive myelosuppression; avoid concurrent use. Antacids containing magnesium and Aluminium reduce ribavirin absorption; take ribavirin 2 hours apart from antacids.
