INDICATION
Ratnox 100mg Cap is a broad-spectrum triazole antifungal indicated for the treatment of systemic and superficial fungal infections including aspergillosis, candidiasis, cryptococcosis, histoplasmosis, blastomycosis, onychomycosis (nail fungus), tinea versicolor, and other dermatophyte infections in immunocompromised and non-immunocompromised patients.
MECHANISM OF ACTION
Itraconazole inhibits the fungal cytochrome P450 enzyme lanosterol 14-alpha-demethylase, which is required for the conversion of lanosterol to ergosterol, the primary sterol component of the fungal cell membrane. Depletion of ergosterol causes accumulation of toxic sterols, impairs membrane integrity and permeability, and ultimately leads to fungal cell death. Itraconazole has a broader spectrum than fluconazole, including activity against Aspergillus species, and its highly lipophilic nature ensures excellent tissue penetration into skin, nails, and lungs.
DOSAGE AND ADMINISTRATION
• Onychomycosis (pulse therapy): 200mg twice daily for 1 week per month for 2 to 3 months
• Systemic fungal infections: 200mg once or twice daily for 3 months depending on infection
• Take capsules with a full meal to maximize absorption (food increases bioavailability by 30%)
• Always follow the dosage and duration prescribed by your doctor
SIDE EFFECTS
Common:
• Nausea, abdominal pain, dyspepsia
• Headache and dizziness
• Elevated liver enzymes
• Skin rash
Less common:
• Hepatotoxicity and liver failure (monitor LFTs in prolonged therapy)
• Heart failure with negative inotropic effect at higher doses
• Peripheral neuropathy with prolonged use
• Hypokalaemia

• Known hypersensitivity to itraconazole or other triazole antifungals

DRUG INTERACTIONS
Itraconazole is a potent CYP3A4 inhibitor and significantly increases plasma levels of many drugs, including statins (rhabdomyolysis risk; avoid simvastatin and lovastatin), cyclosporine, tacrolimus, digoxin, midazolam, triazolam, and many cardiovascular drugs. It is contraindicated with cisapride, astemizole, terfenadine, pimozide, quinidine, and ergot alkaloids. Rifampicin, isoniazid, and carbamazepine reduce itraconazole levels. H2-blockers and PPIs reduce itraconazole absorption by reducing gastric acidity.
