INDICATION

Ranola 500mg Tab is an antianginal agent indicated for the treatment of stable chronic angina pectoris in adults who remain symptomatic despite optimal doses of first-line antianginal drugs such as beta-blockers, nitrates, or calcium channel blockers. It is used as add-on therapy to reduce the frequency of angina episodes and improve exercise tolerance.

MECHANISM OF ACTION

Ranolazine inhibits the late inward sodium current (late INa) in cardiac myocytes, which is abnormally enhanced during ischemia. By blocking late INa, ranolazine reduces intracellular sodium accumulation, which in turn reduces calcium overload via the sodium-calcium exchanger (NCX). This decreases myocardial wall tension and oxygen demand without significantly affecting heart rate or blood pressure, representing a unique hemodynamically neutral antianginal mechanism. The extended-release formulation ensures sustained therapeutic plasma levels with twice-daily dosing.

DOSAGE AND ADMINISTRATION

•      Initial dose: 500mg twice daily

•      May be increased to 1000mg twice daily after 2 to 4 weeks if tolerated and further symptom control is needed

•      Swallow whole; do not crush, chew, or break the tablet

Strong CYP3A4 inhibitors dramatically increase ranolazine exposure, raising toxicity risk; concomitant use is contraindicated. CYP3A4 inducers reduce ranolazine plasma levels significantly, compromising efficacy. Ranolazine inhibits CYP2D6 and P-glycoprotein, increasing levels of drugs such as metoprolol, simvastatin, digoxin, and certain antidepressants; monitor closely. Grapefruit juice increases ranolazine levels and should be avoided.
