INDICATION

Ramacin 250mg Capsule is a macrolide antibiotic indicated for mild to moderate bacterial infections caused by susceptible organisms, including community-acquired pneumonia, acute exacerbations of chronic bronchitis, pharyngitis and tonsillitis, sinusitis, skin and soft tissue infections, and as part of Helicobacter pylori eradication regimens.

MECHANISM OF ACTION

Clarithromycin binds to the 50S ribosomal subunit of susceptible bacteria and inhibits translocation of aminoacyl-tRNA, blocking protein synthesis. It is bacteriostatic at standard concentrations and bactericidal at higher concentrations against certain organisms including Haemophilus influenzae. Clarithromycin has enhanced acid stability and bioavailability compared to erythromycin, allowing twice-daily dosing. Its active 14-hydroxyclarithromycin metabolite is also microbiologically active and has synergistic activity against Haemophilus influenzae.

DOSAGE AND ADMINISTRATION

•      Adults: 250mg to 500mg twice daily for 7 to 14 days depending on infection severity

•      H. pylori eradication: 500mg twice daily with a PPI and amoxicillin or metronidazole for 7 to 14 days

•      Take with or without food; food does not significantly affect absorption

•      Always complete the full prescribed course as directed by your doctor

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea

•      Abdominal pain and dyspepsia

•      Taste disturbances (bitter metallic taste; very common)

•      Headache

Less common:

•      QT prolongation and risk of arrhythmias including torsades de pointes

•      Hepatotoxicity and abnormal liver function tests

•      Clostridium difficile-associated diarrhoea with prolonged use

•      Hypersensitivity reactions including Stevens-Johnson syndrome (rare)

NOT RECOMMENDED IN

•      Known hypersensitivity to clarithromycin or other macrolide antibiotics

•      History of QT prolongation or concurrent use with QT-prolonging drugs

•      Concurrent use with ergotamine or dihydroergotamine (risk of ergot toxicity)

•      Severe hepatic impairment; concurrent use with colchicine in patients with renal or hepatic impairment

DRUG INTERACTIONS

Clarithromycin is a potent CYP3A4 inhibitor and significantly increases levels of many drugs including statins (myopathy risk; avoid simvastatin and lovastatin), cyclosporine, tacrolimus, digoxin, and certain anticoagulants. Concurrent use with colchicine can cause fatal colchicine toxicity, particularly in renal or hepatic impairment; contraindicated in these patients. QT-prolonging drugs must not be co-administered. Rifampicin dramatically reduces clarithromycin levels.
