INDICATION

Plazo 250mg Tab is a macrolide antibiotic indicated for mild to moderate bacterial infections caused by susceptible organisms, including community-acquired pneumonia, pharyngitis and tonsillitis, sinusitis, skin and soft tissue infections, otitis media, and urogenital infections caused by Chlamydia trachomatis.

MECHANISM OF ACTION

Azithromycin binds reversibly to the 50S ribosomal subunit of susceptible bacteria, blocking translocation of peptidyl-tRNA and inhibiting RNA-dependent protein synthesis. It is bacteriostatic at standard doses but may be bactericidal at high concentrations against specific organisms. Its unique pharmacokinetic profile includes extensive tissue distribution, a very long tissue half-life of 68 hours, and high intracellular concentrations, allowing once-daily dosing and short 3 to 5-day courses that maintain therapeutic tissue levels for up to 10 days after the final dose.

DOSAGE AND ADMINISTRATION

      Community-acquired pneumonia: 500mg on day 1, then 250mg once daily on days 2 to 5

      Pharyngitis, skin infections, sinusitis: 500mg on day 1, then 250mg on days 2 to 5

      Chlamydia: 1g as a single dose (four 250mg tablets)

      Always complete the full prescribed course even if symptoms improve

SIDE EFFECTS

Common:

      Nausea, vomiting, diarrhoea

      Abdominal pain

      Headache

      Dizziness

Less common:

      QT prolongation and cardiac arrhythmias (rare; risk in at-risk patients)

      Clostridium difficile-associated diarrhoea with prolonged use

      Abnormal liver function tests or cholestatic jaundice

      Hypersensitivity reactions including rash, urticaria, and angioedema

NOT RECOMMENDED IN

      Known hypersensitivity to azithromycin, erythromycin, or other macrolide antibiotics

      History of cholestatic jaundice with prior azithromycin use

      Patients at risk of QT prolongation or taking other QT-prolonging drugs

      Severe hepatic impairment

DRUG INTERACTIONS

Azithromycin can prolong the QT interval; avoid concurrent use with other QT-prolonging agents including antiarrhythmics, fluoroquinolones, and antipsychotics. Antacids containing aluminium or magnesium reduce azithromycin absorption rate; take at least 1 hour before or 2 hours after antacids. Warfarin anticoagulant effect may be enhanced; monitor INR. Cyclosporine levels may increase; monitor closely.

STORAGE INSTRUCTIONS

Store below 25°C in a dry place away from direct sunlight and moisture. Keep in original blister packaging. Keep out of reach of children.
