INDICATION

Pinaz 10mg is indicated for the treatment of schizophrenia, including the management of positive symptoms (hallucinations, delusions) and negative symptoms (social withdrawal, flat affect). It is also used in the acute management of manic episodes associated with bipolar disorder, and as an adjunct to antidepressants in treatment-resistant major depressive disorder.

MECHANISM OF ACTION

Pinaz contains Olanzapine, an atypical antipsychotic that exerts its therapeutic effects through broad receptor antagonism. It blocks dopamine D2 receptors in the mesolimbic and mesocortical pathways, reducing psychotic symptoms. Simultaneously, it antagonizes serotonin 5-HT2A and 5-HT2C receptors, which contributes to mood stabilization and a reduced risk of extrapyramidal side effects compared to older antipsychotics. It also acts on histamine and muscarinic receptors, accounting for some of its sedative and metabolic effects.

DOSAGE AND ADMINISTRATION

•      Schizophrenia: Starting dose 10 mg once daily; may be adjusted between 5–20 mg/day.

•      Bipolar mania: 10–15 mg once daily; may be adjusted to 20 mg/day based on response.

•      Taken orally once daily with or without food.

•      Swallow the tablet whole with water. Do not discontinue abruptly without medical advice.

SIDE EFFECTS

•      Weight gain, increased appetite, and metabolic changes (blood sugar and lipid levels).

•      Drowsiness, dizziness, or orthostatic hypotension.

•      Dry mouth, constipation, or peripheral oedema.

•      Rare: tardive dyskinesia, neuroleptic malignant syndrome (NMS), or elevated prolactin.

 

NOT RECOMMENDED IN

Contraindicated in patients with known hypersensitivity to olanzapine. Not recommended in elderly patients with dementia-related psychosis due to increased mortality risk. Use with caution in patients with narrow-angle glaucoma, diabetes, liver disease, seizure history, or cardiovascular disease.

DRUG INTERACTIONS

Olanzapine may interact with CNS depressants (enhanced sedation), carbamazepine (reduced olanzapine levels), fluvoxamine (increased olanzapine levels), antihypertensives (enhanced hypotensive effect), and levodopa/dopamine agonists (antagonism). Alcohol should be avoided.
