INDICATION
Petra 40mg Inj is an intravenous proton pump inhibitor indicated for the short-term treatment of gastro-esophageal reflux disease and erosive esophagitis when the oral route is not possible. MECHANISM OF ACTION
Esomeprazole is the S-enantiomer of omeprazole and a selective, irreversible proton pump inhibitor. After intravenous administration, it is activated in the acidic environment of the gastric parietal cell canaliculi, where it forms a covalent sulphenamide bond with cysteine residues on the H+/K+-ATPase enzyme, blocking the final step of gastric acid secretion. Intravenous delivery bypasses gastrointestinal absorption and achieves rapid therapeutic plasma levels, making it the preferred route when patients are nil-by-mouth, critically ill, or post-operative. As the pure S-isomer, esomeprazole has a more consistent pharmacokinetic profile than racemic omeprazole across CYP2C19 metabolizer phenotypes.
DOSAGE AND ADMINISTRATION
• Adults: 40mg once daily by slow intravenous infusion over 10 to 30 minutes
• Upper GI bleeding: 80mg IV bolus followed by 8mg/hour continuous infusion for 72 hours
• Zollinger-Ellison syndrome: 40mg to 80mg twice daily; titrated as needed
• Administer by intravenous route only; switch to oral esomeprazole as soon as clinically appropriate
SIDE EFFECTS
Common:
• Headache
• Nausea, diarrhoea, flatulence
• Injection site reactions with intravenous administration
• Abdominal pain
Less common:
• Hypomagnesaemia with prolonged intravenous therapy
• Clostridium difficile infection risk with prolonged PPI use
• Elevated liver enzymes
• Anaphylaxis (rare)
NOT RECOMMENDED IN
• Known hypersensitivity to esomeprazole, omeprazole, or other substituted benzimidazole PPIs
• Concurrent use with atazanavir or nelfinavir (significantly reduced antiretroviral absorption)

DRUG INTERACTIONS
Esomeprazole inhibits CYP2C19 and has less effect on clopidogrel bioactivation than omeprazole, making it a preferred PPI in patients on antiplatelet therapy. It reduces absorption of atazanavir, nelfinavir, ketoconazole, and itraconazole by raising intragastric pH. Methotrexate renal excretion may be reduced by PPIs; monitor for methotrexate toxicity. Digoxin absorption may be slightly increased with concomitant PPI use.
