INDICATION

P-NEC 20mg Tab is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder with or without agoraphobia, social anxiety disorder, generalized anxiety disorder, and post-traumatic stress disorder (PTSD). It is one of the most widely prescribed SSRIs for anxiety-spectrum and depressive conditions.

MECHANISM OF ACTION

Paroxetine selectively inhibits the reuptake of serotonin into presynaptic neurons by blocking the serotonin transporter (SERT), increasing serotonin availability in the synaptic cleft. Unlike other SSRIs, paroxetine also has significant muscarinic acetylcholine receptor antagonism and mild norepinephrine reuptake inhibition, contributing to both its efficacy in anxiety disorders and its anticholinergic side-effect profile. It has no significant affinity for dopaminergic, histaminergic, or adrenergic receptors at therapeutic doses. Therapeutic effects typically emerge after 2 to 4 weeks as serotonergic neurotransmission normalizes.

DOSAGE AND ADMINISTRATION

•      Depression and anxiety disorders: 20mg once daily in the morning, with or without food

•      Dose may be increased in increments of 10mg at intervals of at least 1 week; maximum 50mg/day

•      OCD and panic disorder: may require up to 40 to 60mg/day under specialist guidance

•      Always follow the dosage and titration schedule prescribed by your psychiatrist; never stop abruptly as withdrawal syndrome can occur

SIDE EFFECTS

Common:

•      Nausea (most common, particularly in the first 1 to 2 weeks)

•      Somnolence or insomnia

•      Dry mouth, constipation, and sweating (anticholinergic effects)

•      Sexual dysfunction: reduced libido, delayed orgasm, erectile dysfunction

Less common:

•      Discontinuation syndrome: dizziness, electric shock sensations, irritability, and flu-like symptoms on abrupt stopping

•      Weight gain with prolonged use

•      Hyponatremia (low sodium), particularly in elderly patients

•      Serotonin syndrome when combined with other serotonergic drugs (rare but serious)

NOT RECOMMENDED IN

•      Concurrent use with MAO inhibitors (risk of serotonin syndrome; 14-day washout required before and after paroxetine)

•      Known hypersensitivity to paroxetine or any excipient

•      Children and adolescents under 18 years (increased suicidal ideation risk)

•      Pregnancy, particularly in the first trimester and near term (neonatal withdrawal syndrome and cardiac septal defects reported)

DRUG INTERACTIONS

MAO inhibitors are strictly contraindicated with paroxetine; a 14-day washout is required in both directions. Paroxetine is a potent CYP2D6 inhibitor and significantly increases plasma levels of tricyclic antidepressants, antipsychotics including risperidone and thioridazine, propafenone, flecainide, and tamoxifen (reducing tamoxifen efficacy). Concurrent use with other serotonergic agents including tramadol, triptans, linezolid, and lithium increases serotonin syndrome risk. Anticoagulant effect of warfarin may be enhanced; monitor INR.
