INDICATION

Oxa-One 600mg Tab is an oxazolidinone antibiotic indicated for the treatment of infections caused by susceptible gram-positive organisms, including vancomycin-resistant Enterococcus faecium (VRE) infections, nosocomial pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA), complicated skin and soft tissue infections caused by MRSA and Streptococcus pyogenes, and community-acquired pneumonia in hospitalized patients.

MECHANISM OF ACTION

Linezolid is a synthetic bacteriostatic antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit at the 23S ribosomal RNA in a unique site distinct from other antibiotics. This binding prevents the formation of the 70S initiation complex required to begin the translation of bacterial messenger RNA, blocking protein synthesis at an early step. Its unique mechanism means there is no cross-resistance with other antibiotic classes, making linezolid effective against multi-drug-resistant gram-positive organisms.

DOSAGE AND ADMINISTRATION

•      Adults: 600mg twice daily, every 12 hours

•      Duration depends on infection site: pneumonia 10 to 14 days; skin infections 10 to 14 days; VRE 14 to 28 days

•      May be taken with or without food

•      Always complete the full course as prescribed by your doctor; use under hospital or specialist supervision for serious infections

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea

•      Headache

•      Taste disturbances or tongue discoloration

•      Elevated liver enzymes

Less common:

•      Myelosuppression: anemia, thrombocytopenia, neutropenia with prolonged use exceeding 2 weeks (monitor CBC weekly)

•      Peripheral and optic neuropathy with prolonged therapy

•      Serotonin syndrome when combined with serotonergic drugs

•      Lactic acidosis (rare but serious; monitor for unexplained acidosis)

NOT RECOMMENDED IN

•      Known hypersensitivity to linezolid or any oxazolidinone

•      Concurrent use with MAO inhibitors (serotonin syndrome and hypertensive crisis risk)

•      Uncontrolled hypertension, phaeochromocytoma, or carcinoid syndrome without monitoring

•      Pregnancy and breastfeeding unless benefit clearly outweighs risk

DRUG INTERACTIONS

Linezolid is a reversible, non-selective MAO inhibitor and must not be used concurrently with serotonergic drugs including SSRIs, SNRIs, TCAs, triptans, and tramadol due to severe risk of serotonin syndrome. Adrenergic drugs including dopamine, adrenaline, and pseudoephedrine may cause exaggerated pressor responses; use with extreme caution. Foods high in tyramine (aged cheeses, fermented products, red wine) should be avoided. Rifampicin reduces linezolid plasma levels.
