INDICATION

Oris 1mg Tab is an atypical antipsychotic indicated for the treatment of schizophrenia and related psychotic disorders, manic episodes of bipolar disorder, and persistent aggression in patients with moderate to severe Alzheimer's dementia. It is commonly used as an initial starting dose during titration in new patients.

MECHANISM OF ACTION

Risperidone is a benzisoxazole derivative that acts as a combined D2 and 5-HT2A receptor antagonist. The 1mg dose is the standard starting dose that allows gradual titration to minimize side effects while initiating therapeutic dopamine blockade. D2 antagonism reduces psychotic symptoms and 5-HT2A blockade modulates dopamine release in the nigrostriatal pathway, reducing the incidence of extrapyramidal side effects compared to typical antipsychotics. Risperidone is metabolized to 9-hydroxyrisperidone (paliperidone), which is equally active.

DOSAGE AND ADMINISTRATION

•      Adults: initial dose 1mg twice daily on day 1; increase to 2mg twice daily on day 2 if tolerated

•      Maintenance: 4 to 6mg per day in most patients

•      Elderly: 0.5mg twice daily initially, increasing slowly

•      Always follow the titration and dosing schedule prescribed by your psychiatrist

SIDE EFFECTS

Common:

•      Extrapyramidal symptoms at higher doses

•      Somnolence and fatigue

•      Weight gain

•      Hyperprolactinemia

Less common:

•      Tardive dyskinesia with prolonged high-dose use

•      Orthostatic hypotension

•      QT prolongation

•      Metabolic syndrome risk with long-term use

NOT RECOMMENDED IN

•      Known hypersensitivity to risperidone or paliperidone

•      Elderly patients with dementia-related psychosis

•      Parkinson's disease or Lewy body dementia

•      Concurrent use with other antipsychotics without specialist guidance

DRUG INTERACTIONS

CYP2D6 inhibitors such as fluoxetine and paroxetine increase risperidone plasma levels. Carbamazepine reduces risperidone efficacy through enzyme induction. CNS depressants have additive sedative effects. Antihypertensives may produce additive hypotension.
